Job Description: We are seeking a Care Coordinator to manage patient care activities, ensuring safe and effective outcomes. The role focuses on care gap management, hospital discharge coordination, chronic disease management, and preventative care, all while collaborating with patients, healthcare teams, and community resources.
Responsibilities Include:
- Identify and assist in the closure of care gaps (e.g., labs, imaging, vaccines).
- Conduct pre-visit planning for annual wellness exams, chronic care and complex care appointments.
- Collaborate with clinic team members to optimize patient health and outcomes
- Coordinate continuity of care with patients and families following hospital discharge.
- Review hospital discharge summaries and coordinate follow-up care.
- Manage care for patients with chronic conditions, coordinating referrals, training, and DME services.
- Monitor complex and acute patient needs, ensuring appropriate care and follow-up.
- Assist in scheduling physical exams, Medicare Wellness Exams, and pre-ordering necessary tests.
- Support quality initiatives and maintain insurance metrics for compliance.
- Participate in quality improvement projects and initiatives.
- Regularly communicate with providers regarding patient care through both verbal updates and the EMR system.
- Maintain patient confidentiality in accordance with HIPAA standard
