On An Average Day You Will:Â
(Includes, but not limited to)Â
- Use electronic medical record (EMR), schedule patient visits according to plan of care and staff availability for nurses and aides
- Use EMR, enter scheduling data and create schedules
- Communicate changes in schedules to staff and patients
- Prioritize urgency of unexpected visit needs and schedule clinical staff
- Provide quality review of visit frequencies and missed visits
- Provide quality reports to the appropriate supervisor and/or case manager
- Assist nursing staff with urgent medication orders, DME needs, etc.
- Accept and perform other related duties and responsibilities
Qualifications:Â
- Must be an LVN/LPN with knowledge of hospice
- Must have a current licensure in good standing with the state
- Must have two (2) years of experience scheduling in a hospice or healthcare environment
- Must have experience with healthcare software programs
- An enthusiasm for providing the best possible care to patients and their families
- Understanding of the hospice philosophy and issues of death and dyingÂ
- Excellent observation, verbal, and written communication skills
