
CNA/PCA/Direct Care Position summary:
The Direct Care Staff will maintain the ongoing personal care for all residents during his or her shift, including completion of special tasks such as activities, laundry and some light housekeeping. He or she will be supervised by the Resident Care Coordinator, Shift/ Unit Supervisor and Director of Nursing. This is an hourly position.
Responsibilities:
- Upon arrival, read the 24-Hour Log back to your last day of work.
- Conduct room checks with previous shift.
- Provide personal care assistance as needed to residents based on the implemented service plan. (Baths, continence checks, escort services, etc.)
- Meal preparation and meal assistance when required including deliver room trays and pickup after meals.
- 30-minute room checks for memory care and 2-hour for AL and document.
- Report changes in resident conditions, document in care suite and 24-hour log.
- Conduct other rooms checks per service plan.
- Answer all call lights and in a timely manner.
- Wear walkie-talkies at all times.
- Complete task sheets, laundry, and general cleaning as needed.
- Contact family members as necessary regarding the resident’s condition. Contacts can be for needed appointment, incidents, needed supplies, etc.
- Help serve meals and clean up, keeping in mind the dietary needs of the residents.
- Encourage residents’ involvement in socialization and participation in activities. Assist with activities as needed or conduct activity. Gather residents for activities and conduct activities with supplies provided when Activity Coordinator is not present.
- Greet all visitors in a courteous manner.
- Staff must be awake and alert during all shifts.
- Always be alert to the emergency medical needs of residents and secure assistance when needed.
- Repair or correct any minor items (toilet paper, light bulbs working, microwave off, etc.).
- Notify the Administrator, Director of Nursing, Resident Care Coordinator or Maintenance person of any faulty equipment or necessary repairs of building in writing. If it is an emergency and the Administrator or the Director of Nursing cannot be contacted at home; call the cell phone.
- Call the Administrator in all emergencies.
- Answer incoming calls in a professional manner. Take messages and distribute them through the staff mailboxes.
- Give tours of the facility if the Marketing Director or Administrator is unavailable. Fill out card when tour is competed and put in HR box.
- Enforce the house rules, which are incorporated into the residents’ lease.
- Respect and honor the residents’ rights as stated in the “Residents Rights and Responsibilities”.
- Update and submit list of supplies needing to be ordered to the Director of Nursing/ Resident Care Coordinator.
- At the completion of your shift, record any “news” in the computer 24-Hour Logas well as complete on the paper 24-Hour Log and sign your name.
- Remain in the facility until the next shift person relieves you. Do a security room check with next shift.
- Complete nursing notes in computer prior to the end of the shift. If computer is not operational, complete the paper Nurses Notes and sign your name. Make sure ADLs are clicked off in care suite. If ADL’s not completed document why.
- Make sure shower logs and skin assessment logs are complete after showers are completed.
- Residents weight and vitals are to be completed by the 5th of each month and recorded.
- Participate in staff meetings.
- Take trash out before leaving and replace trash bags.
- Complete and provide documentation of attending 18 hours of in-service education annually.
- Complete all other tasks as assigned by the management team.
- Exhibit teamwork with all departments.
- When blue dotted staff member must stay over for on coming shift when there is a call out or on coming shift is running late or a no call no show.
