Enhanced Case Manager

Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program.  

As an Enhanced Case Manager with Chartiers Center, you provide direct oversight and personalized and specialized Service Coordination to participants of the Enhanced Supportive Housing Program.   

Job duties include:   

• Coordinate and oversee the delivery of benefits, treatment and support services to clients    

• Independently assess client needs and develop individualized plans that provide the intensity and frequency necessary. 

• Meet with clients on a frequent basis to provide needed interventions and monitor progress 

• Function as an interdisciplinary team member.     

• Participate in rotating on-call to support participants of the program.

Minimum Qualifications:  

• Bachelor’s degree in human services and 3 years of mental health experience OR   

• Master’s degree in human services with 2 years of experience   

• Valid Driver’s License Required
