ESSENTIAL SKILLS AND EXPERIENCE:

Provide direct client services to individuals and/or family members. Services provided may include case management, skills training, group skills training, health promotion, transportation, supportive employment services, counseling with appropriate clinical oversight and others depending on client need. Services may be provided in office, home, school or community settings

Document all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with internal and regulatory standards requirements

Maintain close communication with the consulting psychiatrist and the Behavioral Health team for input regarding medication compliance, side effects of medication, and medication changes and alerts behavioral health team of any changes in client adjustment which might suggest decompensation and a need for more aggressive intervention

Work environment: While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time when traveling to various clinic sites. The noise level in the work environment is usually moderate. The employee may be subject to health hazards (contagious diseases, blood borne pathogens, etc.) when working in the clinic area.
