Purpose of Job:
The purpose of this position is to improve the overall health of the communities we serve by collaboratively working with staff to provide pharmacy services at NEW Health in accordance with policies, procedures and regulations of the NEW Health Board of Directors and administration as listed below.
Essential Duties and Responsibilities:
- Reviews prescriptions issued by a Physician or authorized prescribers to assure accuracy and appropriateness.
- Counsels patients on proper use and storage of medication, side effects, and drug interactions
- Directs pharmacy ancillary staff on matters pertaining to pharmacy
- Reviews patients charts and authorizes prescriptions with delegated prescriptive authority under state approved Collaborative Drug Therapy Agreements.
- Review the appropriateness and effectiveness of patients medications and makes recommendation to providers on medications and disease state management.
- Utilize computer to access patient charts, maintain accurate inventory levels, data entry and verification.
- Maintains knowledge on rules presented by HRSA to maintain the integrity of the 340B Drug Pricing Program
- Assists in the design, development, implementation and maintenance of medication therapy management and clinical pharmacy services.
- Performs other duties as assigned.
- Travel may be required.
