JOB-RELATED DUTIES – ESSENTIAL FUNCTIONS
- Assess program client and participants in a comprehensive evaluation of each client to screen for substance abuse history and/ or issues.
- Document, implement and update the treatment and /or individual service plan (ISP) for clients as assigned.
- Write weekly progress notes.
- Facilitate group counseling sessions including, among others, relapse prevention.
- Participate in quality assurance review as required by program management.
- Plan and facilitate aftercare with community-based providers.
- Identify and develop relationships with community-based providers.
- Develop and deliver training to program staff about identifying and treating substance abuse in clients.
