General Role and Function:

Responsible for the overall care and medical supervision of clients while attending Grace Place. Coordination of medication management and administration with family members, responsible parties, and treating physicians. The LVN may be assigned to any Grace Place center and work schedule may change as needed. This position is responsible to the Activity Director.

 

Key Tasks and Responsibilities:

  • Assess the clients’ nursing and medical needs by working with the family members, responsible parties, and treating physicians.
  • Develop clients’ individual care plan.
  • Obtain physician’s orders for medication and treatments to be administered to clients while at the center.
  • Determine whether self-administered medications have been appropriately taken, applied, or used as directed by the clients’ treating physician.
  • Enter daily or monthly progress notes on medical care provided.
  • Administer medication and treatment as authorized by treating physician.
  • Collaborate with the Activity Director to ensure the activity schedule is implemented. Conduct activities with individual clients and groups as needed.
  • Ensure that Quarterly Caregiver surveys are completed for reporting purposes.
  • Provide oversight to the Certified Nursing Assistants (CNA) as needed.
  • Provide monthly in-service training to direct service staff as required by the Texas Department of Health and Human Services.
  • Maintain medical records of clients and update monthly.
  • Provide health education so the center staff to ensure their awareness of Alzheimer’s disease and other diseases pertaining to seniors.
  • Document all incidents and ensure that all incident reports are maintained on file.
  • Assist with monitoring compliance with licensing requirements and regulations for Day Activity and Health Services facilities.
  • Lead implementation efforts for infection prevention and control.
  • Assist in the development and revision of emergency preparedness and response policies and procedures.
  • Adhere to the policies and procedures of the center and organization.
  • Assist with the orientation, training, and supervision of the center volunteers.
  • Other duties as assigned by management.

Qualifications and Education Requirements:

  • Must have an active LVN license with the Texas Board of Nursing.
  • Must be CPR & AED certified and have a cleared TB test.
  • Must be able to use reasoning abilities to manage multiple tasks simultaneously and prioritize duties and assignments effectively.
  • Bilingual (English/Spanish) and the ability to read and write in Spanish strongly preferred.
  • Must be able to relate in a positive and warm manner to seniors, their families, and responsible parties; showing sensitivity to the needs of those living with dementia.
  • Understand and adhere to confidential requirements.
  • Must have intermediate knowledge of office skills including general computer skills, managing electronic records, filing, faxing, copying, and telephone.
  • Must have reliable, personal vehicle and a valid Driver’s License, good driving record, and applicable insurance coverage required by the company policy.
  • Team player with the ability to work independently.
