Case Manager - Wellness Together

University Settlement partners with 40,000 New Yorkers on the Lower East Side and in Brooklyn every year to build on their strengths as they achieve healthy, stable, and remarkable lives. For 135 years, we’ve collaborated with our communities to pioneer highly effective programs that fight poverty and systemic inequality. Established in 1886 as the first Settlement House in the United States, we bring the values of that movement into the 21st century by meeting New Yorkers where they live, listening to their perspectives, recognizing their excellence, understanding them as complete individuals, and creating space for them to organize. Joining together with our neighbors to advocate for justice and equality, we help build community strength.  

 

Established older adults program provides comprehensive services to the diverse population of the Lower East Side.  These services incorporate the Department for the Aging Health and Wellness model into the daily activities.  Services include congregate and homebound meals, case assistance, social and recreational activities. Ongoing training and professional supervision provided. 

 

Responsibilities: 

  • Perform intake on-going assessment, provide services they need to ensure good health and wellness  

  • Provide outreach, information, case assistance, entitlement evaluation, referrals and advocacy  

  • Document services provided to clients in writing and keep client files up to date; provide regular statistical reporting on outcomes. 

  • Coordinate with Longevity through Education Art Recreation and Nutrition ( LEARN) staff for quality control and integration of services 

  • Assist in designing documents or forms for record-keeping and outreach purposes; assist in program planning and evaluation 

  • Assist with program activities  

  • Complete data entry into appropriate database systems and CRM platforms timely  

  • Attend meetings and professional development trainings 

  • Keep up to date on all information and resources pertaining to older adults 

  • Collaborate with program staff and supervisors  

  • Other duties as assigned by Program Director 

 

Qualifications:

  • Bachelor's degree in social work, Human Services, or related field required 

  • At least one years of experience working with adult population 

  • Strong interpersonal skills including tact, diplomacy, patience, and professionalism 

  • Working knowledge of social theories and practices 

  •  Social perceptiveness and empathy 

  • Ability to build and maintain professional helping relationships 

  • Ability to relate and communicate with diverse population and groups 

  • Resilience along with ability to assess situations 

  • Excellent skills in Microsoft Office 

  • Must have strong attention to detail and able to work independently 

  • Must demonstrate strong commitment to working in a multi-cultural environment 

  • Demonstrated ability to engage hard to reach populations 

  • Excellent verbal and written skills required  

  • Extensive knowledge of NYC benefits programs and advocacy skills required 

  • Bilingual language skills required in either Russian, Spanish, or Mandarin/Cantonese, must be fluent in written and spoken language requirements will change based on needs.   

  • Will be required to obtain CPR/AED certification upon hire 

 

Work Schedule: Full time, Monday - Friday, 9:00am - 5:00pm, 35 hours per week, Hybrid
