Intake coordinator is responsible for screening all potential families into the program, working closely with program administrators to determine eligibility and appropriate case assignments. The Intake Coordinator is the first point of contact for families and referral sources. They will articulate clearly what the program offers and also learn the needs of the families.
Responsibilities:
- Receives and responds to new referrals for clinical services from within University Settlement and outside community-based organizations and provides information about other available resources to referral source or family if needs are not a strong fit for the program.
- Provides intake forms including grievance and confidentiality policies
- Identifies client needs during initial contact with referral source and/or family
- Obtains additional information, if necessary, through telephone and face-to-face contacts as appropriate
- Registers client and intake information into the Electronic Health Record (EHR) system
- Works closely with management to monitor all information and referral activities of clients and families awaiting the assignment of a therapist as necessary
- Maintains progress note documentation, which records nature of intake calls received and action taken
- Collects required documents and verifies client’s insurance
- Updates clients’ information appointments, maintain medical records using EHR
- Participates in weekly supervision
- Other related duties as assigned by supervisor
Qualifications
- Bachelor’s degree required, preferably in Social Work, Human Services, Psychology or related field
- 1-2 years’ experience in a mental health or social services setting
- Computer literacy and professional oral and written communications skills required
- Bilingual (English/Spanish or English/Chinese) preferred.
- Must be adept with data entry and with knowledge of EHR systems.
- Must be self-efficient, attentive to detail, and able to communicate with staff, referral sources, and families with clarity.
- Good communication and organizational skills
Work Schedule: Part time, Flexible schedule, 17.5 hours per week
