POSITION SUMMARY: Personal Care Workers is responsible for providing and assisting participants with activities of daily living and supportive care necessary to the preservation of the home environment. Assists in monitoring the health status and providing a safe environment for participants. Functions as a member of interdisciplinary team (IDT) when needed.

RESPONSIBILITIES AND DUTIES:

ESSENTIAL JOB FUNCTIONS:

  • Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed
  • Assists in monitoring participants health status (vital signs, weight, skin condition, appetite and changes in functional ability). Observes participants taking prescribed medications. Reports pertinent information to Home Care Coordinator in a timely manner
  • Assists participants with IADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) as assigned
  • Documents services provided to participants consistently and accurately via Electronic Health Record (EHR). Follows procedure for flow sheet completion. Documentation is completed in a timely manner
  • Participates in participants care planning through cooperation with multidisciplinary teams
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions
  • Participates in Company required and assigned training programs and any required staff and training meetings
  • Other duties as requested/assigned

Qualifications

  • High school diploma or equivalent
  • Minimum of one year of experience with a frail or elderly population preferred
  • Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers
  • Must have a current Colorado Drivers License and driving record must be in compliance with Company policy. Must maintain current automobile insurance and have reliable transportation
  • Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records
  • Ability to work independently with minimum supervision
  • Knowledge of how to use and maintain in-home equipment (adaptive, hoyer lift, etc.)
