JOB SUMMARY
This position is an integral part of our HIV/AIDS program. This position is responsible for overseeing and carrying out all elements of CTRS as defined below. The position will be based out of designated health centers.
JOB SPECIFICATIONS
Responsibilities/Duties
- Conduct programmatic activities working closely with the HIV/AIDS Priority Clinic team members;
- Identify patients indicated for routine HIV screenings, pre/post-test counseling, risk reduction, and education;
- Administer rapid HIV test to patients who have consented and perform fingerstick confirmatory and baseline testing as indicated;
- Obtain informed consent from patients for HIV testing or documentation of reason for refusal;
- Educate and equip patients and community with risk reduction resources and strategies;
- Order appropriate labs screenings and ensure warm hand-off connection to services;
- Facilitate linkages to care for HIV-positive patients referred from community-based organizations, or self-referred;
- Facilitate linkages to care for HIV-positive patients identified through the AHS Universal Testing initiative;
- Serve as a main point of contact for internal and external PCHP referrals;
- Coordinate with internal and external providers to schedule quarterly follow-up screenings and pharmacy renewals;
- Accurately maintain and track data to generate and submit timely reports to PCHP Project Manager;
- Keep accurate and comprehensive charts on patients, including information on patient interviews, counseling sessions, referrals and HIV counseling and testing;
- Follow-up with patients, health center social workers or service providers and agencies to determine outcome of referrals;
- Establish and maintain relationships with providers to ensure appropriate referrals and outreach;
- Identify venues to conduct outreach activities;
- Participate in HIV priority team, staff and program meetings, site visits with funders and required trainings;
- Adhere to the HIV program harm reduction approach (meeting people where they are and withholding judgment about choices they make; etc);
- Provide services in accordance with the standards set by Ambulatory health Services, AACO and Centers for Disease Control;
- Comply with all program protocols;
- Complete AHS orientation and all mandatory trainings of program to guarantee appropriate competency in program requirements;
- Perform other duties as assigned.
