JOB RESPONSIBILITIES:
Professional Development
*1.Adhere to agreed-upon work schedule, report for work punctually, and assure that absences do not adversely affect the timely completion of responsibilities, and unduly burden co-workers. Accurately record hours worked on timesheet.
*2.Participate in all aspects of the Person-Centered Support Planning (Support Plans) process for persons served. Assist consumers in achieving their desired outcomes through the completion of individual Goals/Action Plans by target dates and document progress. Attend meetings as assigned by the C.O.R.E. Coordinator/Senior Life Enrichment Coordinator/Life Enrichment Director.
*3.Provide transportation to consumers to and from various community settings and operate specialized equipment as needed in a safe and appropriate manner. Maintain acceptable insurance eligibility.
*4.Model appropriate and respectful interpersonal relations during interactions with participants, co-workers, and supervisor. Support everyone in a friendly and caring manner.
*5.Ensure completion and maintenance of all required program documentation, both electronic and written, in accordance with licensing, accreditation, and policy.
*6.Complete required training in order to maintain competency in position.
Communication
*7.Respond positively and professionally to individuals served, co-workers, supervisor, Cottonwood staff, parents, and community stakeholders. Present a positive, fair, and accurate image of the agency to the public and support Cottonwood’s mission.
*8.Practice and promote team participation, negotiation and follow through of assigned responsibilities.
*9.Facilitate intra-agency communication regarding service provision, Cottonwood policy and general organizational happenings between departmental staff and other Cottonwood personnel by ensuring that information is shared accurately and appropriately.
Creating a Meaningful Day
*10.Provide continuous support to ensure safe, clean environments that are conducive to a fun and enriching day that is facilitated in a manner that promotes success.
*11.Serve as a consumer advocate by preserving and promoting the rights, dignity, and opportunities for choice, health, and safety of persons served.
*12.Ensure that the services provided to consumers enhance the independence, and self-sufficiency of the persons served through meaningful activities offered throughout each day. Encourage participants to plan learning and leisure activities by listening to individual preferences, and offering choices and suggestions of activities through monthly program meetings and informally through daily conversation.
*13.Assist in the enhancement of service curriculum. This includes but is not limited to educational, recreational, and leisure classes, local events and outings, Learning Burst activities, and other opportunities for learning and personal growth.
*14.Facilitate relationships with the community as a whole by encouraging friendships, volunteer activities.
Health and Safety
*15.Demonstrate zero tolerance for abusive, neglectful, and/or exploitative acts towards individuals served. Report any suspected incidents according to Cottonwood policies and procedures.
*16.Assist individuals in their C.O.R.E. experience by maintaining work place organization in compliance with licensing, all applicable standards, Cottonwood policy, and the consumer’s needs and abilities with respect to housekeeping, maintenance reporting, record keeping, and documentation. Staff will provide to individuals served the necessary supports to maintain their environment in a safe and healthy manner.
*17.Staff will attend to the personal needs of consumers, including help with meals, toileting/hygiene, and specific health maintenance as necessary.
*18.Maintains competency in delegated wellness tasks as determined by nursing staff and maintains accurate documentation
*19.Participate in and train individuals in services on the proper response to required safety drills.
*20.Provide emergency/crisis intervention as needed for medical emergencies, evacuation situations, behavioral events, etc.

*21.Perform all other projects and duties as assigned by the C.O.R.E. Coordinator.
*22.Demonstrate the five and fifty rule—greeting everyone when they are within five feet and smiling when they are within fifty feet.
*23.Develop caring relationships with each individual you work with using lavish praise and positive reinforcement.
