Job Summary
The advanced practitioner is a licensed independent practitioner who is responsible for managing and coordinating care of patients in accordance with state and federal rules and regulations and the nursing standards of care. The advance practitioner is supervised by a physician.
Responsibilities (include, but not limited to)
- Provides education and patient care management to patients by obtaining a medical history and performing physical assessments appropriate to the patient's presenting complaints and age.
- Analyzes, interprets, and evaluates subjective and objective findings to develop an assessment in order to initiate a treatment plan.
- Develops a plan of care based upon assessment, which includes prescribing appropriate medications and treatments, providing follow-up care, and evaluating the effectiveness of care.
- Promotes healthy living through education and provides patient education regarding medications, treatments, anticipated outcomes, and referrals.
- Provides appropriate documentation in the EHR.
- Attends and participates in staff conferences, clinic meetings, committee meetings, and NP/PA meetings.
- Participates in performance improvement activities of the team or department and demonstrates the ability to further and support the organization’s Mission, Vision, and Values.
- Works effectively as a member of a team. Takes leadership responsibility for completion of tasks when indicated and satisfies team expectations regarding work habits.
