Some of the Responsibilities of a Care Coordinator:

• Communicate daily with field staff regarding schedules and pertinent client information.
• Ensure all physician-ordered care is scheduled and completed.
• Maintain employee/client calendars in the agency database daily, ensuring all shifts/visits are staffed.
• Provide direct patient care when needed. 
• Professionally respond to inquiries from staff, clients, and external stakeholders regarding scheduling.
• Exhibit Iowa Home Care’s Mission, Vision, and Core Values consistently.

• Education: High School Diploma or GED required.
• Experience:
  • Home Health Aide or CNA experience preferred.
  • 3-5 years of scheduling experience preferred.
  • 1-2 years of home care experience preferred.
• Strong knowledge of scheduling principles, organizational methods, and office procedures.
• Proficient with computers, phones, and other office equipment.
• Effective communication skills and the ability to maintain confidentiality.
• Valid driver’s license and proper auto insurance.
