Arnold Oil Company has a long-standing commitment to protecting your privacy and the privacy of plan participants. As you may be aware, recent laws required that we provide you with notice as to how we protect your “nonpublic personal information”. We want you to know that we are guided by our respect for the confidentiality of your personal information and that of your fellow plan participants. The information you provided, or which we obtain when servicing the plan is treated sensitively and held securely in our hands. We are providing you with this notice in accordance with recent laws and, moreover, because we want you to understand that we value your privacy.

You do not need to respond to this notice in any way. It is informational and intended to let you know the strength and commitment to protecting you nonpublic personal information.

Information We Collect

Nonpublic information is any information that we obtained about you in the course of providing you with benefits in connection with your health and welfare plan. The non-public personal information we obtained could include but is not limited to social security number, medical history, employment history, prior coverage with other carriers, etc.

Nonpublic personal information may have been obtained from several sources including:

• Application(s), claim form(s), or other forms you completed in order to enable our TPA to administer our plan;
• The normal procedures involved in processing a claim; or
• Other sources, such as medical information bureaus, motor vehicle reporting agencies, other carriers, agents or brokers, utilization review companies, etc.

Our Privacy and Security Procedures

Arnold Oil Company put privacy and security measures in place to protect your personal information long before nay federal or state laws required us to. We enforce strict control over your nonpublic personal information and limit access to those involved in administering our plan, or otherwise providing a service related to our plan. Arnold Oil Company had developed procedural, physical, electronic and contractual safeguards to secure the confidentiality of information; we evaluate these on an ongoing basis.

Information We Disclose

Arnold Oil Company does not disclose any nonpublic personal information about our employees, plan participants, or former plan participants to anyone, except as permitted or required by law.
