Terros Health is currently hiring an Apprentice-Community Health Worker for our Stapley location 

ASU Students Only!!

Full-Time: 40 hours/Week

 

  • Provides ongoing services as Community Health Worker Intern to patients in need of health care services for the purpose of establishing and reaching appropriate health-related goals. Register patients, collect insurance/demographic data, and schedule.
  • Seeks appropriate clinical supervision to ensure proper skills and knowledge for the position.
  • Provide outreach and engagement to individuals through community events, street outreach and education and training.  This includes dissemination of programmatic health literature and information on obtaining services related to PrEP, substance use disorder treatment, and primary and behavioral healthcare. Provides testing opportunities to targeted individuals by collaborating with internal Terros Health departments such as the HIV/STI prevention programs.
  • Follows up on patient related phone calls and coordinates a response back to patients based on consultation with the care management team.
  • Develops and maintains positive community-based relationships with healthcare partners to benefit patient care coordination.
  • Assists patients with keeping appointments.  Also works to assist patients understand the purpose of visits and the follow-up needed.
  • Facilitates team building with peers by appropriate participation in scheduled staff meetings and trainings; and provides support and leadership to other staff members
  • Participates in departmental assessment of treatment and program outcomes by providing requested information related to patient care.
  • Follows up on missed medical appointments to appropriately reengage the patients.
  • Maintain accurate and confidential records
  • Completes patient documentation within the established guidelines including but not limited to completion registration and progress notes on the date of service, including appropriate Electronic Health Record (EHR) documentation
  • Ability to drive to company sites and patients’ homes throughout the state of Arizona.  Driving may be required more than 50% of work time

Qualifications

  • ASU CHW Program Required
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.
  • Promotes, models and uses strength-based engagement and treatment strategies for all patients served.  Lends their unique insight into behavioral and/or physical health recovery.
  • Excellent interpersonal skills both in person and by phone, with high professionalism.
  • Excellent communication skills.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 20 working days of assuming role.
  • Must pass a TB and Drug Screen Test
