Terros Health is pleased to share an exciting and challenging opportunity for an ACT Specialist in our Lifewell South Mountain clinic in PhoenixThis person will function as a Case Manager on the Assertive Community Team (ACT).  The individual will need to be professional, friendly, a self-starter, organized, and compassionate.

Responsibilities:

Terros Health is hiring an ACT Specialist for our South Mountain clinic in Phoenix, AZ. 

Additional $3.00/hour ACT differential pay 

Now Offering a $4,000 Sign on Bonus

 

Full-Time, Employed

Bachelor’s Degree in Behavioral Health preferred (not required) 

State Peer Support Certification required - we can help you obtain this upon hire! 

 

Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

Must have very flexible schedule as this team provides treatment 7 days/week, including weekends and holidays

On-call and mileage reimbursement pay available 

 

As an ACT Specialist you will provide services to our clients, to include the following:

  • Coordinate, monitor and provide direct services in a community setting for clients served on the ACT team
  • Work as a member of a clinical team to assess client needs
  • Participate in the development of services plans, provide/coordinate services related to the service plan
  • Act as a client advocate, coordinate services and discharge plans, and assist in crisis intervention
  • Provide on-call crisis intervention services, as well as weekend and holiday coverage

 

Qualifications

  • High School or equivalent (Bachelor’s or Master’s degree in Behavioral Health preferred)
  • 3+ years in the delivery of behavioral health services required
  • 3+ years of Case Management or ACT specialist experience preferred
  • Valid Arizona Driver’s License
  • Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members
  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
  • Basic understanding of the DSM-V
  • Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred
