Description

We are a mission-driven, service-orientated industry leader looking for an ACT Housing Specialist in Phoenix, AZ.  You will be working on an interdisciplinary Assertive Community Treatment (ACT) clinical team working with individuals diagnosed with a Serious Mental Illness or General Mental Health diagnosis under Court Ordered Treatment. We are seeking a passionate individual to complete the team in our outpatient Healthcare Center in Phoenix, AZ. This individual will need to be professional, friendly, organized, compassionate and have a good understanding of housing resources within Maricopa county.

Responsibilities:

Seeking an ACT Housing Specialist to join our interdisciplinary ACT clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our 23rd Ave Healthcare Clinic. 

 

$4,000 Sign-on Bonus!

 

Full-Time Employed: 40 hours/week

Monday - Friday 8:30am - 5:30pm

Location: 23rd Ave and Dunlap in Phoenix, AZ 

Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

Full Benefits Package, including 401K

Must have very flexible schedule as this team provides treatment 7 days/week, including weekends and holidays

Generous PTO/Sick Time (4+ weeks in year 1)

Quarterly Stay Incentive Available

Starting at $19/hr

$2/hour ACT Differential in ADDITION to base pay, plus rotational on-call stipend

 

  • Provides direct treatment, rehabilitative and supportive services that support independent living in a community setting to members.
  • Works in conjunction with the patient and ACT team staff to assess patient needs and wants, assist the patient in locating housing of their choice, assists in accessing affordable, safe, decent, and permanent housing.
  • Supports patients in protecting their tenancy rights regardless of participation in ACT team services.
  • Participates in the development of individual service plans, acts as a patient advocate and liaisons with families and community agencies.
  • Provides on-call crisis intervention services, as well as weekend and holiday coverage. (schedule flexibility required) 

$$ ACT incentives of an additional $2/hour, on-call stipends and mileage reimbursement

Qualifications

    • HSD/GED or equivalent education required.
    • Preferred - 3 years experience in the delivery of behavioral health, or
    • Bachelor's in Behavioral Health or related field with 1 year of behavioral health experience, or
    • Master's Degree in Behavioral Health field
  • 3 years working as a housing specialist preferred - minimum of 1 year required 
  • Must have a good understanding of housing resources within Maricopa county    
  • Must be 21 years of age or older, with a valid Arizona driver’s license, and a minimum of 4 years driving experience and meet requirements of Terros Health’s driving policy
  • Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for one within 7 working days of employment.
  • Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
  • Basic understanding of the DSM-V
