Description

The Stroke Nurse Coordinator is responsible for organizing and administering the stroke outreach programs. This position coordinates stroke education programs to agencies in the Onslow Memorial Hospital and Onslow County referral areas. The Stroke Nurse Coordinator provides the Stroke Risk Identification Screening Program to the public; develops, implements and maintains Beyond the Hospital Programs for stroke survivors. The Coordinator assures all documentation of outreach activities as required by various agencies.   

 

The Stroke Nurse Coordinator shall work collaboratively with patients and their families, health care providers, community groups, and schools to facilitate Stroke Care and prevention throughout the continuum of care.  The Stroke Nurse Coordinator will coordinate regional stroke screening outreach activities that unitizes principles of program management; advanced clinical care and leadership to effectively meet customer needs; and the goals of the grant.  Providing leadership and system planning for participating hospitals; coordinating community screenings, securing screening sites, personnel, volunteers and equipment.  The Stroke Nurse Coordinator collaborates with hospitals and professional organizations to facilitate the development of the stroke program: assessing stroke community screening needs; participating in meetings as appropriate to assure effective communication; demonstrates effective interpersonal communication skills; provides expertise in the area of stroke specialization; consults with health care institutions and community agencies to meet the needs of the patients, volunteers, and care providers. Stroke Nurse Coordinator shall develop and promote a standard of care consistent with decreasing inpatient resource utilization, and contribute to service excellence at the hospital and outpatient level in accordance with the goals established in the guidelines of the Kate B. Reynolds Charitable Trust, NC Stroke Association and NC Collaborative Stroke Registry and Onslow Memorial Hospital policies and procedures. 

Qualifications

Education/Certification:

Bachelors or Associates Degree in Nursing required, MSN preferred.  

Must be a licensed Registered Nurse in North Carolina.  

 

Experience:

Minimum of two years experience in Neurologic Critical and or emergency care setting and evidence of advanced clinical knowledge, leadership teaching abilities, and communication skills is preferred.

Previous experience in case management, stroke care and knowledge of community resources is desired.
