EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated part time Case Manager to join the Career Services Program.

 

Salary: $25 hour

Schedule: 19 hours per week. Mon, Tues. and Thurs. Available for morning and afternoon hours.

Location: Central Islip, NY

Reports to: Program Supervisor

 

 

What drives the core purpose of this job:

The Part-time Case Manager within our Safe Harbour program is responsible to provide short and long term, trauma informed, case management and direct services to minor victims of human trafficking and youth at-risk of human trafficking, to ensure that victims receive the most comprehensive response.

 

 

What are the key responsibilities:

  1. Provide trauma informed, victim centered case management services and outreach to minor victims of human trafficking and youth at-risk of human trafficking to youth in Suffolk County, NY.
  2. Conduct assessments for client needs and develop an individualized service plan to meet such needs.
  3. Transport and escort clients to court proceeding, medical appointments and other appointments as needed.
  4. Assist with applying for public benefits, employment, medical care, return of personal property and short- and long-term coordination of such services.
  5. Make appropriate referrals for medical exams, mental health services, residential assistance
  6. Using an evidence-based curriculum the social worker will facilitate group services to support youth who have been trafficked or are at risk of being trafficked.
  7. Document in the CAC database the services required by the client to ensure accurate case records are maintained. 
  8. Maintain accurate database of professional referral resources that assist stated population.
  9. Provide back-up and support to CAC for office coverage.
  10. Prepare monthly and quarterly statistical and annual reports as required by funding source.

 

 

What key knowledge, skills, and abilities are required to excel in this role:

  1. Bachelor’s degree in Social Work, Forensic Psychology or behavioral health field required, Master’s preferred.
  2. Working knowledge human trafficking, commercial sexual exploitation of children, child abuse and trauma.
  3. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
  4. Excellent communication, organizational and interpersonal skills.
  5. Must have access to automobile and possess valid NYS driver’s license
