Case Manager

 

EAC Network, a not-for-profit social service agency that empowers, assists, and cares for 62,888 people in need through 100 programs across Long Island and NYC, seeks a Case Manager to work full time for its Nassau County Family Treatment Court program.

The Case Manager position is a non-exempt position paying $30 per hour at 35 hours per week. The hours of operation are Monday-Friday 9:00AM-5:00PM and the office is located in Westbury, NY. The role reports to the Program Supervising Case Manager.

Primary Purpose of Job:

The Case Manager is responsible for Court Liaison, assessment and referral of substance abusing clients involved in Family Court into appropriate treatment programs.

 

Principal Duties & Responsibilities:

  1. Act as liaison to the Family Court System.
  2. Conduct screening and assessment interviews and arrange for client to be placed in appropriate treatment program and provide trauma informed case management.
  3. Maintain complete client records.
  4. Prepare and present written status reports to the Family Court System regarding client's progress.
  5. Investigate and arrange supportive services for clients.
  6. Escort clients to treatment facilities when indicated transporting in own vehicle if necessary.
  7. Maintain a caseload of substance abusing clients.
  8. Complete all evidence-based assessments for appropriate assessment on clients
  9. Make visits to treatment agencies assigned to caseload.
  10. Identify and link clients to supportive services.
  11. Have understanding of and refer to internal EAC programs, such as New Path, Health Home, CASA, Supervised Visitation among others.
  12. Conduct urine, breath and hair follicle testing, maintain appropriate documentation and submit findings to court.
  13. Install, monitor and maintain secure remote alcohol monitoring devices and submit findings to the court.

 

Knowledge, Skills, & Abilities Required:

  1. Bachelor’s degree in Social or Behavioral Science, Criminal Justice or CASAC and minimum of two years’ related experience.     
  2. Good communication skills, both oral and written.
  3. Ability to work cooperatively with treatment and Family Court System.
  4. Must have access to automobile and possess valid NY State Driver’s License.
  5. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.
