Service Coordinator – The Haven on Lincoln
Summary of Position: The Service Coordinator will assess clients’ 5 social determinants of health to plan for individualized services through home, community, and office visits. They will provide individually-based motivational interviewing, trauma-informed care, and support to clients to assist them in increasing their quality of life and recovering from mental illness. The Service Coordinator will advocate for clients and link them to community resources. They will provide client-centered, strengths-based, trauma-informed services to clients experiencing barriers, including mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach. The Service Coordinator will provide support services in the community. He/she/they will complete case management tasks with clients within a shared caseload. The Service Coordinator will be responsible for documenting all services provided to clients according to agency policy and state requirements.
Position Type: This position is exempt and reports to the Manager of Resident Services

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the development of a comprehensive individual service plan with the client, client’s support system, and other care providers.
• Provide in-unit services to clients in a variety of community hotels, motels, and extended stay locations.
• Facilitate discharge planning with participants and interdisciplinary team members.
• Engage with clients to increase financial independence utilizing all forms of income supports (i.e. benefits and employment)
• Provide housing and benefits navigation services to all assigned clients.
• Assist clients in learning and improving independent living skills; i.e. personal hygiene, housekeeping skills, nutrition, medication compliance and management, and shopping for food and personal items
• Educate and assist clients with applying for entitlements; i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and SNAP
• Accompany clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, and social support.
• Ensure all clients have a pathway to permanent housing that meets their unique needs.
• Advocate on client’s behalf and empower clients to advocate on behalf of themselves when appropriate
• Complete all required documentation in a timely manner.
• Perform other related duties and/or projects as assigned

Other Duties:
As assigned by the Director of Resident Services.
Work Environment: This position operates in a professional office environment; may also be required to travel throughout community utilizing personal vehicle or other methods of transportation. The hours of work are generally Monday through Friday, 9:00 AM to 5:00 PM, although evening and weekend hours, including holidays, may sometimes be required.
Physical Demands: Physical demands are a part of the essential functions of this position, especially as it relates to the capacity to move freely around the community in a vehicle, transport clients, etc. Reasonable accommodations for the physical demands of the essential functions of the position can be considered and made when possible in response to a specific request. Must be able to lift and move with up to 50lb.
