The Clinical Admin will provide administrative support to primary care teams in facilitating patient care and promoting positive health outcomes.
. Duties will include:

Patient Care Coordination in triaging calls and My Chart Messages, routing to appropriate team members and closing communication loop with patients as needed.

Supporting Quality Measures, ensuring patients are current with all Health Maintenace Goals and follow up visits, ensuring access to care.

Bridging barriers to care, including arranging for transportation, translation services, coordination with outside agencies and completing paperwork required.

Documenting and updating computer systems (EPIC, Sorian) to order and coordinate patient services
