ACCOUNTABLE TO: Registered Peer Supervisor (for Direct Support Professional Duties), PRP BHH Department Director / Rehabilitation Specialist (for Direct Support Worker Duties), SHPFM Department Director / Assisted Living Manager (for Alternate Assisted Living Manager / House Manager Duties)

 

CLASSIFICATION: Part-time.  24 hours per week. W-2.

 

WORK SCHEDULE:  Thursday - Saturday from 4:00 PM to 1:00 AM each day

COMPENSATION RANGE : Compensation ranges between $15.00 - $18.00 per hour; and is commensurate upon experience, expertise, company budget, and verified credentials.  

 

ANTICIPATED TRAVEL: 20% of the time. There is no additional compensation for travel.

 

SUMMARY OF POSITION RESPONSIBILITIES:  The worker will be responsible for managing the day-to-day operations of the housing unit as a guest of the client for the residency in which the assigned client's reside.  The worker will help to manage clients' treatment schedules for outpatient mental health clinic and primary care services, operate nurse practitioner originating site equipment for telehealth based psychiatric, and help clients participate in quality assurance sessions.  Structured direct support service encounters are to take place during your shift as needed and documentation submitted daily.   Worker may need to be available to assist client's in the event of a crisis.  Worker's shall ensure client's receive all prescribed medications while monitoring the client's ability to self-administer / secure those medications frequently throughout the month providing assistance where necessary.  Clients cope with co-occuring mental health and substance abuse disorders or have a history or substance abuse; for these clients, worker will be asked to collect urine samples from client on a regular basis and process those samples with the program's preferred processor.

The Company is a community mental health and addictions treatment service provider that tailors specialized therapeutic treatment services to emotionally and behaviorally challenged youth, adults, and families.  Our Psychiatric Rehabilitation Program provides community-based services that improve the quality of life for individuals with mental health challenges and social impairments.  Our strengths-based rehabilitation services are designed to produce healing and restoration in communities.   A direct support worker is expected to facilitate psychiatric rehabilitation, behavioral support services, and crisis intervention for assigned clients, according to Individualized Rehabilitation Plan, by:

  • teaching and instruction

  • encouragement of positive change

  • skill demonstrations and practice

  • promote individual wellness, self-management, and recovery

  • assist in the restoration and/or improvement of self-care social skill

  • assist with access to entitlements

 

SCHEDULED DUTIES AND RESPONSIBILITIES:

  1. Provide direct support worker services and certified peer recovery services to assigned clients as scheduled by the Company (supervisor).  Caseload shall not exceed 45 clients.  Must document each encounter in ICANotes on the same day of the encounter by the close of business. Worker may set schedule to meet each client independently.

  2. Be on duty either in the office or assigned location Monday to Friday 8a-5p and available on call to clients after hours one hour break is from 12-1 each day

  3. As a guest of the client, one who is helping the client to manage their day to day lives independently, collaborate the day to day housing operations and property management needs on the property in which your group living clients reside

  4. Attend staff meetings once per week or as scheduled

  5. Responsible for carrying out services and supports that meet the clients medically necessary mental health needs reflected in the client's individual treatment & rehabilitation plan (ITRP) promoting independence, freedom of choice and overall wellness

  6. If requested by supervisor, collect urine samples from each client as medically necessary

  7. Collaborate with the client's therapist, nurse case manager, mental health case manager, psychiatrist and other persons involved in the client's care to ensure the client complies with treatment schedules on a monthly basis (in some cases weekly)

  8. In collaboration with the client’s entitlement case manager, assist the client with monitoring and obtaining access to all entitlement benefits for which the client may be eligible and monitor benefit status on a monthly basis helping the client to apply when necessary

  9. Be available to clients in the event of a mental health crisis while on duty

  10. If the client resides in a community based group living arrangement, assist the client with needs as it pertains to said arrangement in addition to other psych rehab needs

  11. Carry out any other work related functions requested by the supervisor

  12. Ensure clients have an adequate supply of medications and monitor medications on a regular basis for each client

  13. Document incidents, hospital admissions in contact note and incident report

  14. conduct room inspections daily

  15. Participate in team meetings as scheduled

  16. you may need to transport clients in company vehicle, if so follow company policies and procedures relating to the matter

  17. You will collaborate regarding your schedule with the Rehabilitation Specialist and ensure you complete the following additional duties:(a) manage the tele-medicine system when clients in your caseload are scheduled to receive treatment via tele-medicine with OMHC providers; (b) collect urine/saliva samples from clients for medication and substance abuse monitoring as needed; (c) be available during community housing unit inspections and ensure clients manage housing units according to posted policy; (d) receive food and supply deliveries; (e) assist registered nurse case manager; (f) collaborate with client's therapist and all persons involved in the client's treatment at least monthly

  18. Are responsible for implementing the rehabilitation activities outlined in the adult or minor's ITRP.

  19. If assigned on an hourly basis in an assisted living facility, also serve in the capacity of "Alternate Assisted Living Manager" while on duty providing the client with support with activities of daily living and instrumental activities of daily living as it pertains to the assisted living service plan.

  20. If assigned on an hourly basis in a residential treatment facility or supportive housing facility, also serve in the capacity of "House Manager" while on duty.


 

UNSCHEDULED DUTIES AND RESPONSIBILITIES:

  1. Participate in external and internal audits/surveys (CARF/CSA/OHCQ) as directed by the supervisor.

  2. Participates in quality assurance and performance improvement plans by completing periodic audits or other activities to ensure regulatory compliance and/or improve service delivery.

  3. Assist the supervisor with any work-related tasks as requested.

  4. Collaborate with and serve as program POC for external treatment providers and outside agencies as needed.

  5. Responsible for following regulations of COMAR, CARF, any other regulatory body, and company policies/ procedures related to service delivery and documentation and co-facilitating orientation of all new hires to these as well.

  6. Support the maintenance of a safe environment by participating in training and drills as requested.

  7. Advise the supervisor on the development and implementation of protocols to better enhance the efficiency of day to day operations effecting change when approvals are made.

  8. Troubleshoot and abate any issues that could adversely affect the day-to-day operations.

  9. Report to the Supervisor daily and as requested. Report deviation of operational standards to CEO daily.

  10. Collaborate with department leaders, external landlords, vendors, utilities providers, housing related external entities and supervisees regularly as needed.

  11. Have knowledge of Policies and Procedures of Holbrock Estates Supportive Housing / LightHouse Behavioral Healthcare.

  12. Maintain a working knowledge of COMAR as it pertains to assisted living and other residential regulations within the scope of services rendered by the company.

  13. Report to the ALM, CEO, COO, manager via email when starting and ending service period if not using Company’s time keeping system.

  14. Check emails and company group texts at least every 30 minutes while on duty; respond accordingly.

  15. Assist the ALM, CEO, COO and/or contract manager with other services as requested / directed.

PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drive a motor vehicle.

 

WORKING CONDITIONS: Some work is performed in an office.

Qualifications

COMPETENCIES AND SKILLS:

  1. Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job

  2. demonstrate medication handling, infection control and personal care competencies to the Registered Nurse periodically

  3. Excellent knowledge of case management principles, healthcare management and reimbursement

  4. Previous experience with psychological aspects of care

  5. Effective communication skills

  6. Excellent organizational and time management skills

  7. Familiarity with professional and technical emerging knowledge

  8. Problem solving skills and ability to multi-task

  9. Compassionate with teamwork skills

  10. Current license, certification or registration

  11.  Preferred but not required BS degree in related healthcare field

  12. 45 -55 wpm typing; must demonstrate typing skills.

  13. Ability to apply the concepts of customer service in the workplace. Ability to use good professional judgment. Detail oriented. Ability to work independently, with minimal direct supervision.

  14. Posses a basic knowledge of all office equipment, including word processors, typewriters, Dictaphone, FAX, and copy machines, and a willingness to attend appropriate classes to increase knowledge as needed.

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS:

 

  1. It is preferred (but not required) that the candidate possess at least an associate degree in counseling, psychology, social work, or health related field (Interns and students pursuing such degrees may provide services under this scope through contract with the higher learning institution only)

  2. Within 30 days of hire, must be listed as a Certified Medication Technician on the Maryland  or a reciprocal state board website (once hired, if your status is anything other than ACTIVE, we can assist with abating the issue) 

  3. 1-year experience in case management, mental health counseling or substance use disorder related counseling

  4. CNA / GNA preferred but not required.

  5. High School Diploma/GED is required

  6. If servicing minors: 1. A bachelor's degree in a health-related field; 2. An associate's degree in a health-related field; 3. 30 hours, or their equivalent, of college credit toward a bachelor's degree in a health-related field; or 4. 1 year of work experience in a supervised mental health setting;  Are supervised by a licensed mental health professional or a rehabilitation specialist; ii. Have 60 hours of on-the-job direct PRP supervision before providing services to minors served by the program without direct supervision

  7. Minimum two (2+) plus years of experience providing direct support to adults with severe persistent mental illness and substance abuse disorders.  These will be verified through your professional references provided.

  8. Current first aid and CPR certification (the Company can assist with obtaining this once hired)

  9. Must demonstrate you are free from TB, measles, mumps, rubella and varicella (the Company can assist with obtaining this once hired)

  10. Must pass a CJIS criminal background check and have no criminal convictions or history that indicate behavior which pose a potential threat to clients and staff

  11. Ability to follow CWC Clinic practice guidelines – including adhering to the CWC immunization policy and the CWC Mission and Vision and CWC Corporate Compliance Policy.

  12. Must be 18 years or older

  13. Once hired, must complete 40 hours specialized PRP training before servicing clients independently and an additional 20 hours direct supervision if rendering services to minors
