CLASSIFICATION: W2 employee or 1099 independent contractor (if certain qualifications are met)

 

COMPENSATION RANGE: $3,600.00 per month

 

WORK SCHEDULE: Monday-Friday, 8 AM - 5 PM

 

ACCOUNTABLE TO: PRP BHH Department Director

 

ACCOUNTABLE FOR: N/A

 

ANTICIPATED TRAVEL: 0% there is no additional compensation for travel.

 

SUMMARY OF POSITION RESPONSIBILITIES: This position is office based during initial onboarding/ orientation, weekly meetings with supervisor and during rendering of service. It pays $50 per month per client (2 telehealth at the assigned office/ telephone visits each month which generally take 15-30 minutes each call). Each visit may be completed at the main office or via telehealth at the assigned office. There are currently a total of 45 clients on the caseload who need to be serviced monthly.

 

SCHEDULED DUTIES AND RESPONSIBILITIES:

As a Registered Nurse Case Manager (aka Health Home Care Manager), you will be responsible for providing comprehensive and coordinated care to patients. Your primary duties will include:

  • Conducting patient assessments and developing individualized care plans
  • Ensuring compliance with medical records documentation and HIPAA regulations
  • Collaborating with healthcare professionals to ensure the delivery of quality patient care
  • Monitoring patient progress and making necessary adjustments to the care plan
  • Educating patients and their families on disease management and treatment options
  • Coordinating and facilitating communication between patients, healthcare providers, and other stakeholders

PHYSICAL DEMANDS: Ability to work in patient areas that may require bending, lifting, and standing. Capable of handling potentially difficult situations with patients or their families.
 


WORKING CONDITIONS: Office based and Telehealth at the assigned facility or office.

LEVEL OF EDUCATION / TRAINING / QUALIFICATIONS:

  • Bachelor’s degree in Nursing, Social Work, Health Administration, or a related field (Master’s degree preferred)
  • Relevant certification or license (e.g., RN, LCSW) and [insert relevant number] years of experience in healthcare management or home care
  • Demonstrated experience in case management and home care services
