COMPENSATION RANGE: $3,600.00 per month
WORK SCHEDULE: Monday-Friday, 8 AM - 5 PM

ACCOUNTABLE FOR: N/A
ANTICIPATED TRAVEL: 0% There is no additional compensation for travel.
SUMMARY OF POSITION RESPONSIBILITIES: This position is office based during initial onboarding/ orientation, weekly meetings with supervisor and during rendering of service. It pays $50 per month per client (2 telehealth at the assigned office/ telephone visits each month which generally take 15-30 minutes each call). Each visit may be completed at the main office or via telehealth at the assigned office. There are currently a total of 45 clients on the caseload who need to be serviced monthly.
SCHEDULED DUTIES AND RESPONSIBILITIES:
As a Registered Nurse Case Manager (aka Health Home Care Manager), you will be responsible for providing comprehensive and coordinated care to patients. Your primary duties will include:
- Conducting patient assessments and developing individualized care plans
- Ensuring compliance with medical records documentation and HIPAA regulations
- Collaborating with healthcare professionals to ensure the delivery of quality patient care
- Monitoring patient progress and making necessary adjustments to the care plan
- Educating patients and their families on disease management and treatment options
- Coordinating and facilitating communication between patients, healthcare providers, and other stakeholders
