About the Program
The Family Treatment Rehabilitation Program (FTR) located in Brooklyn NY, offers intensive preventive services to families at risk of removal of children to foster care due to neglect and abuse. Risk factors for families served by the FTR program include mental health and substance use. Using Solution Based Casework, we actively work with families to develop plans that make them stronger, and keep them together and to address the Mental health and substance use issues impacting the family. We encourage families to build connections to positive supports in their community who will help them sustain their progress and can be in place after our services discontinue. Our goal is to partner with families to keep them together while ensuring that children will be safely cared for. We utilize a collaborative team approach to provide services to families and ensure families progress through the three Treatment phases of FTR.
Job Summary
The Family Team Conference (FTC) Facilitator is responsible for facilitating and tracking all conferences as required by the Administration for Children’s Services (ACS) utilizing the Family Team Conference Model. The facilitator will ensure the safety and well-being of each child is assessed through a strength-based lens. Utilizing strong engagement skills, the facilitator will yield positive safety and permanency outcomes for children and families with the input from a multidisciplinary team and various stakeholders. This position also supports the beginning engagement with families by facilitating conferences, assisting with Early Engagement, referrals and JHV’s. In the Family Treatment and Rehabilitation program, this position also facilitated the Clinical Diagnostic Team Meetings (CDT's).
Job Responsibilities

Welcoming clients and providing a trauma informed atmosphere for clients while facilitating conferences in the field, the office or virtually.

Facilitate Family Team Conferences: Holding conferences (Initial/Planning/Service termination) on time as per deadlines, and when appropriate; and maintaining fidelity to the Family Team Conference Model and the FT/R model.

Facilitate up to three conferences per day which may be in a family’s home. Identify, evaluate and address any and all safety and risk concerns during conferences.

Hold pre-conferences and debriefing and make visits to assess families if needed.

Schedule and coordinate with program staff, families, FSS and ACS as needed;will cover for a CP if program needs support.

Attend Early Engagement conferences and Joint Home Visits as extension of the program and engage families in services.

Assist in writing of referrals and sending out to FTR team.

Complete administrative responsibilities: including the timely holding of, and documentation of family team conferences in PROMIS, CONNECTIONS and OneDrive ; FTC documentation in Connections by 72hours/PROMIS by end of week.

Follow Family Team Conference schedules and communicate with appropriate ACS and program staff.

Generate tracking reports to track and distribute to program management to ensure all conferences are completed on time.

Monitor outcomes related to identifying performance trends, best practices and opportunities for improvement; Conduct Quality Assurance activities related to Family Team Conferencing (i.e.,monitor the quality of the FTC summaries and outcomes).

Collect statistical data on Family Team Conferences and compile and present reports on a monthly basis.

Participate in all Clinical Diagnostic Team (CDT) meetings and be an active member of an interdisciplinary treatment team to facilitate the client recovery and rehabilitation processes.

Participate in individual and group supervision, staff meetings, training and client meetings as appropriate.

Responsible for ensuring adherence to all agency policies, procedures and governmental regulations,including but not limited to HIPAA.

Provide ongoing education and training to clients and staff on Family Team Conferencing.

Collaborate and strengthen partnerships with community representatives to ensure participation and involvement in conferences.

Provide support to Program Director/Program Supervisor/Clinical Coordinator to ensure that program is following OCFS and NYC Children’s Services’ policies and protocols.
