Description

Under the direction of the Associate Vice President of Nursing Services, Nurse Supervisor is responsible for ensuring the quality of medical, nursing and other health-related services that are provided to individuals with I/DD in all HeartShare day and residential programs in collaboration with their families and consistent with the philosophy of person-centered planning. Provides clinical supervision to nursing staff as well as implements appropriate nursing care practices, standards, and procedures. Ensures that the services are delivered in a manner appropriate to individual needs, including cultural and religious preferences, individual strengths and unique interests. 

he duties and responsibilities of the Nurse Supervisor Utilization Review will include, but not limited to the following:              

  • Oversee training and ensure competencies. Assist in the supervision, training, and professional development of program RNs, LPNs, and AMAP staff. 

  • Provide guidance and direction today and residential Program Coordinators/Managers on strategies and techniques for effective oversight of medical services provided to the individuals in their programs.   

  • Assist in providing training and direction to residential staff regarding the management of medical emergencies/crisis. Monitor staff training initiatives in health-related areas to ensure that staff are appropriately trained and kept up to date. Provide individuals, families, and staff with health education.  

  • Assist in teaching AMAP classes. 

  • Undertake appropriate follow-up activities with assigned nurses, including training and professional development activities. Ensure that nursing staff are well versed in curricula presented in health-related training programs and that nursing staff oversight and direction in programs is consistent with training curricula.  

  • Recommend and implement nursing care practices, standards, and procedures. 

  • Assist with the development of new as well as existing nursing treatment plans.  This may include training of program staff in plan goals and execution. 

  • Remain current in best practices via review of appropriate literature and professional development activities (webinars, trainings, etc.). Keep abreast of changes in the field.  

  • Act as liaison between programs and outside medical providers to ensure individuals receive the best medical care available.   

  • Consult with and serve as a liaison to community providers.  This includes hospital health care personnel during inpatient stays, including but not limited to admissions and discharges.  Monitor and record progress on hospitalized individuals. Participate in interdisciplinary team meetings during hospitalizations and coordinate plan of discharge for individuals served. Recommend hospital discharge plan of care, oversees implementation and follows up to ensure compliance.  

  • Facilitate the resolution of family concerns regarding necessary and appropriate treatment in difficult cases as delegated by Sr. Director of Nursing. 

  • Help to develop and oversee internal systems and policies as needed and agreed upon by Associate Vice President of Nursing Services, Quality Assurance, and Senior Management. 

  • Conduct frequent, periodic medical reviews to ensure quality of medical care provided.  

  • Assist programs to prepare for internal and external audits. 

  • Act in the capacity of the program nurse when there are program vacancies.  This includes participating in team meetings. 

  • Respond to emergency calls as per phone tree. 

  • Assist in recruitment, hiring, orientation, performance evaluation, discipline, and discharge of assigned nursing staff. 

  • Ensure that all medical equipment used by nursing staff is in excellent working order.  

  • Serve as a standing member of agency-wide committees including DD Risk Management, SRC, HRC, and Informed Consent committees. 

  • Assume leadership role with respect to health-related matters. 

  • Function as a resource to agency on health-related matters.  

  • Follow any other job-related instructions and perform any other job-related duties requested by any person authorized to give instructions and assignments, and especially the Associate Vice President of Nursing. 

Qualifications

Education and/or Experience 

  • Associate Degree in Nursing; bachelor's degree preferred. 

  • At least three years of experience as a registered Nurse in the delivery of community-based health services to individuals with developments and intellectual disabilities.  

  • At least one year of supervisory experience. 

  • License and current registration as a RN in the State of New York.  

Reasoning Ability 

  • Ability to analyze and interpret reports and documents.  

  • Ability to respond to common inquiries, concerns or complaints from employees and customers.  

  • Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables.  

  • Ability to resolve conflicts and mediate stressful situations.  

  • Basic understanding of statistics. 

  • Valid NYS Driver’s License and maintain it in good standing.
