HeartShare is looking for a Family Team Conference Facilitator (FTC)

 

Primary Responsibilities and Essential Functions of Position:

  • Facilitate Family Team Conferences (FTC) assessing permanency, safety, and well-being.
  • In preparation for conferences, review FASP, progress notes associated with the review period, court action and previous FTC summaries.
  • Manage the monthly FTC schedule, ensuring FTCs are scheduled, that outreach letters are sent to all family team conference participants, scheduling space for conferences, and facilitating any other special needs requests.
  • Collaborate and coordinate with case planning team and other departments to ensure participation in FTCs.
  • Serve as FTC liaison between HSVS, ACS, & external stakeholders.
  • Responsible for monitoring and ensuring that all agency-facilitated FTC documentation is entered into CONNECTIONS with 5 days of the conference date, copy of FTC summaries is distributed to participants and SPR tab completed.
  • Ensure that documentation reflect the current circumstances of the family and that the action plan/next steps are SMART goals formulated.
  • Responsible for Quality Improvement activities related to Family Team Conferencing (monitors the quality of the FTC summaries and outcomes).
  • Provide ongoing education to clients and staff on Family Team Conferencing.
  • Additional duties as assigned, based on the needs of the agency.
  • Knowledge of NYC Children’s Services Family Team: 1 year (Preferred)

Qualifications

Requirements:

  • BSW, BA , BS, Master’s preferred plus a minimum of 2 years’ experience in Child Welfare, preferable Foster Care.
  • Knowledge of NYC Children’s Services Family Team Conferencing Model
  • Strong clinical skills and strength-based perspective.
  • Strong organizational, interpersonal, written and verbal communication.
  • Ability to accommodate flexible work schedule including evenings.
  • Ability to travel within New York City for in person conferences
  • Strong mediation skills and ability to redirect, summarize, etc.
  • Bilingual in English and Spanish a plus.
