Job Summary

The Family Case Planner will work in our East Flatbush location serving children and families in CD 15, 17 and 18 in a child welfare Prevention program to prevent children from being place outside their home.Providing casework counseling using the Solution Based Casework model with families; home visits and home assessments required and carrying 12 cases. Job requires heavy accountability including detailed progress notes, Family assessments and service plans, conferences, attending trainings, case consultations, staff meetings and weekly clinical supervision.

Job Responsibilities

  • Direct Services are provided to clients via casework, individual, family, and group counseling, as indicated by individual case assessments.
  • Assist in Intake and Crisis Intervention
  • Conduct home visits and attend client-related meetings as needed.
  • Develop client assessments and service plans following the model outlined by Improved Outcomes for Children, Family Team Conference, and SBC.
  • Monitor and assist in service provision to all program clients as required by program mandates.
  • Maintain current case records and service plans (FASP) for families served utilizing Connections and PROMIS systems.
  • Participate in weekly supervision, case conferences and Family Team Conferences as directed by Supervisor and Program Director.
  • Provide parent education and other support services utilizing group format as determined by client need.
  • Participate in training appropriate to the development of enhanced Social Work skills as required by Children’s Services and HeartShare St Vincent’s Services.
  • Assist in day-to-day operations as required by the Program Director.

Qualifications

  • Bachelor's degree required in social work or related field
  • Previous child welfare experience a plus
  • Bilingual Spanish and English required
  • Required to make home & office visits
  • Good written and verbal communication skills
