General Description

 

The Housing Navigator Lead for the Community Supports Program builds bridges between underserved individuals, community resources, and the housing navigators. This position is passionate about the MedZed mission and embodies the STRIVE values daily. This is a remote-field worker who is primarily out in the community serving members and supporting the housing navigator team with program requirements. The HN Lead will have 50% training and administrative duties, and 50% Housing Navigation work based on the standard housing navigator job description. The HN lead will maintain a panel based on the needs of the business. The HN Lead will meet with members and must deliver culturally appropriate information regarding the availability of health and community resources that will reduce barriers to care and is required to act independently in the community under minimal supervision. The HN Lead is from the community of the members & HN’s to be served and is someone who does not give up easily, is self-motivated, conscientious, empathic, and a creative problem solver.

 

 

Responsibilities

  •  Assist the Community Supports Program Manager in managing the overall status of the team  within their market.
  • Conduct the training and development of new hires once out of the initial training with the goal of helping the new hire be independent within 2 months of starting with MedZed.
  • HN Lead will maintain a panel of 30-35 billables and may increase based on the needs of the business with an idea goal of up to 50%.
  • Attending all mandatory meetings
  • Supporting with morning huddles when needed
  • Support CS Program Manager for review on pending assignments and team development.
  • Assist with holding working blocks with HN’s to promote continued coaching opportunities and ensure enrollments & reassessments are completed within the program requirement.
  • Assist with covering 1 on 1 meetings with HN’s and end of day meetings as needed when CS PM is out of the office.
  • Assist with completing back-office tasks as needed such as (team reminders, tracking consents & disenrollment's, transfers, reassessments, reviewing salesforce accounts for accuracy, and graduations for bonuses.
  • Assist with ride a longs and coaching team members on a variety of needs including but not limited to: Salesforce, Salesforce Maps, New hire support, team development presentations, and Provider portals support.
  • Continuously expand knowledge of community resources, services, and programs available to members and build ongoing relationships with these organizations to advocate for members.

  • Working knowledge of housing issues, social issues, and health issues.
  • Ability to quickly establish trust and build strong relationships.

  • Sound judgment and the ability to quickly analyze situations.
  • Ability to establish priorities and meet deadlines.
  • Ability to problem solve in a proactive, creative manner.

  • Must be willing to travel as needed

 

Physical & Other Requirements:

  • Daily travel to local geographic regions.
  • Depending on the region, may require frequent walking.
  • Must comply with work at home policies.
  • Must hold a valid driver's license with an insured and reliable mode of transportation.
  • Flexibility to work evenings and weekends as needed.
  • Ability to perform the essential job functions consistently safely and successfully with the ADA and other federal, state, and local standards, including meeting qualitative and/or quantitative productivity standards.
  • Ability to maintain regular, punctual attendance consistent with the ADA and other federal, State, and local standards.  
  • Must be able to lift and carry up to 50 lbs.
  • Must be able to talk, listen and speak clearly on the telephone.
  • Pass a criminal background and drug testing check.
