Status:                                 Full Time; Regular; Exempt

Location:                            West 140th Street, New York, NY 10031

Days/Hours:                       Monday to Friday, 9am to 5pm

The candidate must have a history of working with a population who have been diagnosed with serious mental illness (SMI) and co-occurring disorders. In addition, the person must be able interact and work with a person impacted by the justice system and have a history of homelessness. Demonstrated ability to work in a team environment.

Summary

Under the Director’s supervision, the Program Manager ensures that case management services for the program are rendered as expected by our funder’s scope of services.

 

 

Essential Duties and Responsibilities:

 

  • Oversee day-to-day operations of Case Managers, this includes individual and group supervision. Providing development, annual evaluations and performance plans as needed to sure quality professional measures are met.
  • Assist the Senor Director/ Director to maintain, develop and implement strategic operational systems that will enhance programmatic reports and evaluation based on funding goals.
  • Monitor intake of new referrals, coordinate with property management to review housing stock.  Review Housing Quality Standards (HQS) to communicate repair needs and maintain quality housing,
  • Review client charts to ensure quality and detailed documentation, provide feedback in supervision and monitor progress.
  • Lead staff unit meetings, provide coverage on other contracts as needed.
  • Act as liaison with members Fortune staff to ensure continuity of treatment from internal and external providers.
  • Assist in creating a culture of support for justice impacted adults.
  • Research, evaluate, and maintain community linkage agreements with external service resources to assist in housing stability.
  • Assist the Senior Director with hiring, training and onboarding of new staff.
  • Develop extensive base of referral agencies that provide the following services: mental health, health, dental care, substance abuse treatment, education, employment, family services, alternatives to incarceration, housing and gay/lesbian/transgender counseling etc.,
  • Submit weekly and monthly reports to the Senior Director in a timely manner,
  • Submit timely monthly and weekly reports on program progress.
  • 24-Hour on call support for emergencies
  • Perform other duties as needed.

 

Supervisory Responsibilities:

The Program Director will oversee the daily functions of the Justice Involved Supportive Housing (JISH) contract, while assessing the program requirements, and assisting with establishing program systems.   

 

Qualifications

 

Qualifications:

  • Bachelor’s Degree required, master's degree preferred.
  • Knowledge of complex contracts, program start-ups and assisting with policy and procedure development.
  • Ability to apply metrics to review work performance towards funding goals. Knowledge of how to use data to inform program needs.
  • Review program spending and make recommendations based on annual budget.
  • Five years of supervisory experience required, at least two years must include complex housing contracts with program deliverables.
  • Experience with case management documentation. Demonstrated ability to detect staff needs based on monthly document review.
  • Preparation with program audits.  Proven record of leading team performance towards successful program goals.
  • Experience with databases for entering client information,
  • Strong oral and written communications skills required. Experience completing program contracts and staff corrective action plans.
  • Bilingual English/Spanish a plus; and

Travel Requirements: This position requires weekly on-site availability and travel to program locations as required. On-call responsibility for emergencies and coverage.
