SHIFT SUPERVISOR / CHARGE NURSE
JOB DESCRIPTION
POSITION PURPOSE AND SUMMARY
As charge nurse you are delegated to the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
RESPONSIBLE TO: Director of Nursing
QUALIFICATIONS: 1. Graduate of an accredited school of Nursing, Current Licensure in State of CA, BLS / PALS Proficiency

The Charge Nurse/Supervisor uses an interdisciplinary approach with residents, their families, support departments, etc., to adequately plan for the needs of patients. They perform specific shift responsibilities.
BASIC DUTIES AND RESPONSIBILITIES
- Count all controlled substances at the beginning and end of each shift.
- Make assignments at the beginning and end of each shift.
- Receive shift report 7:00am, 3:00pm, 11:00 pm from off going shift.
- Verify that keys have been turned in by outgoing shift.
- Evaluates resident health status as well as health care services by making frequent resident rounds. Documents informative entries in the medical record (chart and care plan).
- Check MARS for meds to be given; treatments to be done on own assigned patients.
- Administer meds and treatments as prescribed by physician and document patient’s response to medications and treatments on own patients.
- Assists Nurses as needed to assure the delivery of timely resident care.
- Provides direct supervision and evaluation of work performance of Nurses C.N.A.’s
- Provide nursing care for assigned patients.
- Communicates with physicians’ changes of condition or updates of patient’s condition.
- Rounds with physicians as appropriate.
- Monitor ancillary departments, i.e. lab and radiology for timely completion of test, assessment of results and MD notification.
- Oversee and assist in lunch relief.
- Provide relief for nursing staff as necessary.
- Attends in-service education and staff meetings.
- Assures medications refill are ordered as necessary
- Serves as a resource for all staff including RNs, LVNs, CNAs, & RTs
- Assures 24-hour nursing report is completed for their shift.
- Facilitate disposal of outdated meds.
- Ensure maintenance of crash cart readiness.
- Ensure emergency preparedness by promoting active participation in drills etc. by all staff.
- Reviews audits done by pharmacy and medical records and supervises staff in taking appropriate action to correct deficiencies in a timely manner.
- Verify fax sent on STAT meds needed from pharmacy. Receive and verify any meds delivered by pharmacy.
- Ensures compliance with infection control practices by all personnel under his/her supervision.
- Reviews and completes Dietary recommendations of nutritional assessments.
- Responsible for accurate observations, evaluation and report of resident symptoms.
- Assists nursing personnel with treatments/emergencies and provides direct nursing care to critically ill residents as needed.
- Assist with education of residents, families and personnel as needed.
- Night Shift Charge Nurse or designee reviews all orders of assigned patients received from previous 24 hr. and audits for accurate and complete noting of orders. This includes treatment/medication transcriptions, care planning, nursing notes, etc.
- Coordinates nursing care with ancillary services.
- Protects the resident, his/her privacy and property.
- Check MARs, TARs, UDAs.
- Take sick calls. Attempt to get replacement. May delegate this to Unit Clerks if available.
- Performs special projects on an ‘as needed basis’.
- Performs any other duties as may be requested or assigned by DON.
