POSITION SUMMARY
The Certified Diabetes Care and Education Specialist (Registered Dietitian), will provide nutrition assessment, counseling, group education and Medical Nutrition Therapy (MNT) to diverse populations; develop nutrition education material and curriculum; coordinate wellness programs and group visits; assist in quality improvement programs; implement grant-related activities; and train college externs/interns during community nutrition rotations.

DUTIES AND RESPONSIBILITIES
Direct Patient Care / Clinical Responsibilities
1. Provide nutrition assessment and counseling services to patients, to include: assessment of patients’ nutrition status; development of care plans; and provision of MNT.
2. Work with multidisciplinary team of healthcare practitioners to deliver nutrition services, health education, and chronic disease management.
3. Provide Diabetes Self-Management Education to patients with diabetes and pre-diabetes, including assisting providers with insulin instruction and titration.
4. Ensure accurate documentation of all patient encounters in the Electronic Health Record (EHR) within 72 hours of service delivery.
5. Manage incoming nutrition referrals to ensure timely follow-up and coordinated care.
6. Member Care: Demonstrate understanding and apply working knowledge of safety policies and ensuring safe member practices.

Nutrition & Fitness Programming
7. Coordinate, evaluate, and lead health education programs, including but not limited to group medical visits and the weekly health education series.
8. Coordinate the retinal photography program.
9. Develop and share patient educational materials with patients, staff, and the community.
10. Collaborate with other NAHC departments and programs like WIC, Integrated Behavioral Health, Youth Services, and School Based Health Centers through program and curriculum development, and ensuring access to services for all NAHC’s members.

General Duties
11. Work with the Media Department to promote Nutrition and Fitness (NAF) events and programs.
12. Represent NAF in NAHC’s Wellness Committee meetings.
13. Provide trainings and nutrition/health education to staff and community partners.
14. Act as a preceptor for dietetic interns who come to NAHC for rotations
15. Assist with objectives, deliverables and data management as required by grants, contracts and departmental goals and objectives.
16. Attend trainings and meetings as requested by supervisor.
17. Employee Safety: Safely performs all duties; follows required protective protocols to ensure personal safety as well the safety of others.
18. Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.
19. Quality Improvement: Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.
20. HIPAA: Keep all protected health information (PHI) confidential and abide by HIPAA policies for the release and disclosure of any PHI. Will report unauthorized use of disclosure of PHI immediately, to supervisor or HIPAA security officer.
21. Work well under pressure, meet multiple and often competing deadlines.
22. At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the community.
23. Other duties as assigned by Supervisor.

Qualifications

MINIMUM QUALIFICATIONS
1. Bachelor’s Degree in Nutrition, Dietetics or related field, with an additional one-year dietetic internship.
2. Registered by the Commission on Dietetic Registration or RD eligible.
3. Diabetes Care and Education Specialist certification from the Certification Board for Diabetes Care and Education (CBDCE).
4. Competency in Spanish
5. Must be able to work outside of normal business hours as needed.
6. Willingness to travel between Oakland and San Francisco Clinics.
7. Well-adept at motivational interviewing and behavior change counseling.
8. Sound knowledge of MNT and nutrition counseling, particularly around chronic diseases such as diabetes, hypertension, hyperlipidemia, and obesity.
9. Excellent computer skills and proficiency in Microsoft Office.
10. Excellent organizational skills and superior attention to detail.
11. Ability to communicate effectively with different audiences.
12. Cooking skills and a passion for sharing healthy meal preparation with others.
PREFERRED QUALIFICATIONS
1. Fluency in Spanish
2. One year of experience working in a public health setting.
3. One year of experience providing Diabetes Self-Management Education (DSME), or current status as a Certified Diabetes Educator
