Description

The pharmacist works as part of a cohesive and innovative team to provide culturally respectful patient- centered health care and medications to a diverse population of underserved, houseless and/or low-income adults, in a Federally Qualified Health Center.  This position provides opportunities for both providing and developing clinical pharmacy services as well as performing dispensing pharmacist duties at the Outside In’s in-house 340b pharmacy located in the downtown Portland Outside In Medical Clinic.

 

Essential Pharmacist Duties

 

  1. Provide Clinical Pharmacy Services

 

  • Provide Collaborative Drug Therapy Management (CDTM) services pursuant to established CDTM agreements and Oregon Board of Pharmacy rules.
  • Once credentialed and privileged to perform CDTM duties, provide patient assessment, medication adjustment/changes, medication safety screenings, disease state and medication education, and cost containment/formulary alignment for patients with a referral from their Outside In PCP.  Common disease states treated by Outside In clinical pharmacists include T2DM and HCV.
  • Participate in further development of Outside In’s clinical pharmacy program and CDTM agreements in collaboration with the Medical and Pharmacy Director.
  • Deliver medication therapy management (MTM) services (as defined by APhA) in coordination with the prescribing medical provider for identified Outside In patients.
  • Assist with transitions of care, medication reconciliation as requested by provider. 
  • Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.
  • Research and evaluate professional literature in response to questions and requests. Provide pharmacy consultation to physicians or other health care professionals when requested or appropriate, e.g., cost effective drug alternatives.
  • Precept undergraduate pharmacy students and pharmacy residents providing clinical services during their rotations at Outside In.

 

 

 

  1. Perform Dispensing Pharmacist Duties
  • Prepare and dispense medication orders per provider requirements according to established policies, procedures, and protocols.
  • Interpret and evaluate medical provider's written and verbal orders for accuracy, completeness and appropriateness.
  • Recognize therapeutic problems, including drug interactions, contraindications, improper dosing, precautions of therapy and toxicities. Consult with practitioner when questions or problems are identified.
  • Perform Drug Utilization Review (DUR) on every prescription. Monitor medication adherence, appropriate timing of refills, and acknowledge notifications from third party payors.
  • Provide patient counseling on all new prescriptions and respond to patient questions relating to their medications. Adapt counseling approach based on patient’s individual health literacy and comprehension.
  • Supervise pharmacy technicians and interns while staffing the pharmacy.
  • Report adverse drug reactions, drug defects, and medication errors according to procedure. Perform or supervise performance of daily drug preparation, labeling and distribution

 

  1. Contribute to cost containment initiatives, inventory management, and overall pharmacy compliance

 

  • Accountable for pharmacy inventory including cost containment, handling of controlled substances, tracking edits to on-hand quantities, and expired or recalled medications.
  • Ensure compliance with regulatory requirements of the 340B program.
  • Adherence to Outside In medication formulary and the process for off-formulary requests.
  • Ensure compliance with third party billing practices.

 

  1. Participate in Quality Improvement for the department

 

  • Contribute to the quality and effective operation of the department.
  • Support Continuous Quality Improvement (CQI) efforts. Provide evidence of continued competence to practice pharmacy.
  • Participate in committee work (which may include at least one of the following: Pharmacy and Therapeutics (P&T) committee, Hepatitis C workgroup, Chronic Care QI subcommittee).

 

  1. Support the organization as needed.

Qualifications

Knowledge and Skills

  Preferred Skills

  • Contemporary Ambulatory Pharmacy medication management skills.
  • Professional knowledge of pharmacy operations in FQHC setting.
  • Drug therapy knowledge as described in Essential Duties (dosage, indications, side effects, etc.)
  • Working knowledge of federal and state regulations that impact pharmacy operations, including HIPAA, OSHA, and State/Federal laws governing pharmaceutical services, and HRSA.
  • Demonstrates a caring, respectful and compassionate attitude towards all people

Education and Experience

  • Experience providing clinical pharmacy services in a clinic setting required.
  • B.S. Pharmacy or Pharm D from accredited pharmacy school is required.
  • Experience dispensing medications in an outpatient pharmacy preferred
  • Currently licensed to practice pharmacy in the state of Oregon. Preceptor license preferred
  • PGY pharmacy practice residency desirable
  • Medication Therapy Management certification desirable
  • Bilingual English/Spanish desirable
  • Successful completion of an approved orientation to standard competencies within 6 months of hire

 

 

 

 

Working Conditions

This position is located in a typical office and clinic environment, with moderate noise.

Physical Requirements

  • Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Repetitive use of hands and fingers; stooping; bending; sitting; standing and/or walking for extended periods. Talking and hearing, standing, sitting and walking is required on a regular basis.
