Description

Service “Patient” Navigator works under the direct supervision of the Program Director/Project Coordinator.   The Service Navigator serves as the central hub of services and referrals for clients and treatment staff.  The Service Navigator will fulfill assigned roles; community liaison, administrative support, MAT education and care navigation.  The Service “Patient” Navigator will work alongside the treatment team.   

 Major Tasks, Duties and Responsibilities 

  •  Conducts intakes, screening and education for MAT Projects. 
  • Maintains client files in accordance with all applicable federal, state, local, agency laws, and protocols.  
  • Collects and analyzes client demographic data and provide monthly reports to treatment and frontline staff. 
  • Monitors documentation of services provided in accordance with funder requirements and company protocols.  
  • Attends all required funder, agency, and departmental meetings/trainings. 
  • Provide administrative support to frontline and executive staff. 
  • Monitor and input data including intakes, follow-ups, and discharges. 
  • Ensure all intake interviews, discharges, and follow-ups are completed and entered within timeframes into indicated data collection system.  
  • Work collaboratively with Substance Abuse Counselors, Mental Health Counselors, and Outreach staff to refer clients to appropriate resources and services. 
  • Maintain a continuous calendar of MAT Clinics, either via coordination with Health Centers or On-Site. 
  • Maintain and update a working list of community referrals for client services. 
  • Maintain contact with active clients throughout project and provide referrals and service navigation as needed. 
  • Provide Naloxone /Overdose Prevention education to staff and clients 
  • Knowledge of motivational interviewing and counseling.  
  • Serves as AICS, Inc. representative at resource fairs, stakeholder meetings, and special events throughout the community. 

Service Navigator Competencies and Performance Expectations 

 All Service Navigators are expected to: 

  •  Conducts intakes, screenings and assessments for The Sierra Project. 
  • Identify a crisis 
  • Identify personal boundaries and be able to work as part of the treatment team 
  • Adhere to professional standards 
  • Recognize personal biases working within diverse populations 

Qualifications

Prerequisite Qualifications 

 

  • The following are the qualifications required to perform the essential functions of this position.  Qualifications may be subject to modification based on the Americans with Disabilities Act. 

 

  • Bilingual abilities may be required within certain AICS programs.  

 

  • A bachelor’s degree in a social service related field, or 2 years of experience in providing services similar to those contained within this job description. Experience working with diverse racial/ethnic populations and LGBT individuals. General knowledge of Medication Assisted Treatment, substance use disorders, risk behaviors and local service community, motivational interviewing techniques, and substance abuse service milieu. 

 

  • Must have valid California driver’s license and liability insurance if driving personal vehicle on AICS business. 

 

  • Vision, hearing, manual dexterity and eye-hand coordination must be adequate for performance of job duties.  Able to sit at desk, use keyboard, write and physically perform other job duties.  Able to move about the facility to observe clients and staff.

_____ Class “B” California Driver’s License. 

 

_____ Class “C” California Driver’s License 

 

_____ Personal vehicle with liability insurance for reimbursable mileage, generally short distances. 

 

_____ First Aid certification, obtain within 90 days of employment and maintain current thereafter. 

 

_____ CPR certification, obtain within 90 days of employment and maintain current  

thereafter.  

 

_____ Medication training required within 30 days of employment.

_____ Develop computer skills adequate to perform word processing duties on indicated    EHR’s within 90 days of employment.
