Mountain View Hospital is looking for a RN Care Coordinator to join our team!
JOB SUMMARY:
Performs expert coordination skills in management of high risk maternal fetal medicine patients while working collaboratively with physician and multidisciplinary teams. Provides direct patient care in clinic, patient triage, patient education, and assists physician in the assessment of patient needs and treatment plans. Communicates with providers about changes in patient’s clinical condition, including results of diagnostic studies. Participates in performance improvement activities and quality assurance.

DUTIES AND RESPONSIBILITIES:
- Acts as one point of contact for patients, physicians, and care providers throughout the patients care with MFM clinic.
- Responsible for answering multiple telephone lines and taking care of the caller’s needs quickly and correctly.
- Answers patient inquiries over the telephone, assessing the severity of the patient’s symptoms and medical needs under the MFM physician’s direction. Documents information provided during triage in patient medical record.
- Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care.
- Chart preparation for referred diagnosis including obtaining medical records and labs.
- Assists physician with examinations, procedures and other processes related to direct patient care.
- Participates actively in department problem solving, coordinating patient schedules for multiple areas.
- Schedules services such as tests, procedures, ultrasounds, follow-up visits and any other services the patient may require from the physician.
- Monitors and ensures accuracy of recording on patient medical records. Maintains all required reports, records, statistics, etc.
- Provides health care instruction/education to patient and family members.
- Provides notification to referring providers regarding follow up care and discharge planning.
- Performs a variety of nursing administrative duties.
- Provides care, education and support for patients and families as needed in times of stress or crisis.
- Communicates appropriately and clearly to provider, co-workers and administration.
- Demonstrates and ability to be flexible, organized and function under stressful and emergent situations.
- Maintains established departmental policies and procedures, objectives, performance improvement program, safety, environmental and infection control standards.
- Interacts professionally with patient/family and involves patient/family in the formations of the plan of care.
- Maintains a good working relationship both within the department, with other departments, and with other facilities.
- Written and computer documentation meet current standards and polices.
- Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, staff meetings and workshops; shows responsibility for own professional practice in ongoing education. Attends of staff meetings.
- Demonstrates the ability to use Healthstream and completes assigned modules.
- Demonstrates the ability to access the Polices and Procedures on the Hospital Intranet.
- Performs other related duties as assigned or requested.
