Mountain View Hospital is looking for a Practice Transformation Specialist to join our team!

 

JOB SUMMARY:

The population health practice transformation specialist is involved in supporting MVHs initiatives toward clinical integration and accountable care strategies to achieve the triple aim in healthcare: 1) improving quality and patient satisfaction, 2) improving outcomes and the overall health of the population, and 3) reducing the total cost of care. The practice transformation specialist will monitor trends in overall health and wellbeing of the populations we serve. You will be involved in planning for the implementation of best practices in population health with the end-goal of improving patient health outcomes while decreasing healthcare spending within our organization and affiliated clinics. Our objective for the practice transformation specialist is to build strong relationships with providers in the community and our affiliates and have an extensive knowledge of the contracts held by our organization in an effort to ensure we exceed quality and financial goals.

DUTIES AND RESPONSIBILITIES:

1. Review bi-weekly/monthly reporting to track MIPS and ACO measures.
2. Review CCM and Annual Wellness Visit outreach reporting for all clinics.
3. Understand all contract requirements and regulations in order to be a resource to the clinic staff for related questions.
4. Collaborates with other members of the Population Health Team and Clinic Manager to provide the clinic with resources, ideas, and suggestions on implementation.
5. Understand outpatient clinic operation and build relationships with staff and providers in the clinics.
6. Provide outreach for all clinics in need such as
a. Mailers
b. Phone calls to patients
c. Scheduling for AWV
d. Materials and education
e. Create ideas to help with outreach
7. Assist with data entry and chart audits to close care gaps.
8. Review, monitor, and engage clinics in care gap closure for all payers.
9. Provide support to Care Coordinators within the clinic setting.
10. Participates in webinars, conferences, educational meetings, and transitional calls.
11. Collaborate with Charge Master to get codes added, updated, deleted, etc. in EMR’s as needed.
12. Collaborate with all specialty clinics on MIPS reporting and their roles in the ACO.
13. Collaborate with Point of Care for all Cerner related documentation being added, updated, or deleted as necessary.
14. Understand and help implement all contractual obligations for the ACO, Blue Cross, and Medicaid VCO and assist with educating the population health team on these needs.
15. Solid understanding of HCC coding and quality measures for all payors.
16. Continue any outstanding projects and work through them.
17. Offers solutions to road blocks while implementing processes.
18. Interact professionally with clinic and ancillary staff members.
19. Demonstrate knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served. Able to interpret data about the patient’s status in order to identify each patients age specific needs and provide care needed by the patient group.
20. Support measurement and system improvement.
21. Consult other departments as appropriate for an interdisciplinary approach to the patient’s needs. Collaborates with population health team members in recommendations for change as appropriate.
22. Communicate appropriately and clearly to manager, co-workers and physicians.
23. Maintain a good working relationship both within the department and with other departments/affiliations.
24. Identify and address opportunities for optimized clinic growth and utilization impact.
25. Ensure all deadlines are met.
26. Maintain patient confidentiality at all times. Report to work on time and as scheduled, complete work within designated time.
27. Wear identification while on duty, use computerized punch time system correctly.
28. Complete in-services and return in a timely fashion.
29. Attend annual review and department in-services, as scheduled.
30. Attend at least 75% of staff meetings annually.
31. Represent the organization in a positive and professional maanner.
32. Actively participate in performance improvement and continuous quality improvement (CQI) activities.
33. Comply with all organizational policies regarding ethical business practices.
34. Flexible in scheduling, work weekends and holidays and work overtime when needed.

35. Maintain flexibility in a growing environment and be willing to grow, change, and accept new duties as the scope of urgent care changes with the needs of the community and hospital.
36. Any other duties as deemed necessary by manager

Qualifications

Education/Certifications: Bachelor’s degree in related field preferred

 

Experience: 1 year required healthcare experience, 2-5 years healthcare experience preferred

 

Equipment/Technology: Experience with Word, PowerPoint, and various EMR systems. Proficient in Excel and Microsoft Office.

 

Language/Communication: Ability to read and interpret documents such as quality measure implementation guidelines and requirements as well as utilization reports. Ability to write routine reports and correspondence. Ability to speak effectively before groups or employees of the organization. Must be patient, courteous, compassionate, and respectful with co workers, physicians, and administrative staff. Have the ability to work in a dynamic environment where numerous projects occur at the same time, organization and attention to detail is a must. Can understand input from the supervisor and/or manager concerning errors and instructions. Ability to communicate medical terminology and relate well with physicians, staff and management.

 

Mathematical Skills: General mathematic skills.

 

Mental Capabilities: Can concentrate for long periods of time in order to complete assigned tasks with a better than average attention span in order to listen, read and remember verbal and written instructions. Can understand input from the supervisor and/or manager concerning errors and instructions. Ability to deal with problems involving several concrete variables in standardized situations. A clear understating of the importance of patient confidentiality and sensitivity regarding complex health problems. Ability to use critical thinking skills to assess patients and intervene as needed within scope of practice.

 

Interpersonal: Ability to work well with others as a team player. Organized, detail oriented, and the ability to multi-task while maintaining a heavy load of various tasks. Maintain a neat and professional appearance and workspace.

 

Performance: Must be able to handle conflict issues with staff. Must follow established recommendations and guidelines. Must meet deadlines as needed.
