Mountain View Hospital is looking for a Health Information Management (HIM) Clerk/RHIT to join our team!

 

JOB SUMMARY: HIM/ Medical Records Clerk is responsible for managing, maintaining, and ensuring the accuracy, confidentiality, and accessibility of patient medical records. This includes organizing and retrieving patient charts, ensuring all documentation complies with legal and regulatory standards, and safeguarding patient information. The department is also responsible for verifying that all required medical forms are properly completed and filed. In addition, the team collaborates with healthcare providers and other departments to facilitate efficient patient care while adhering to hospital policies and privacy regulations.

Reviews medical records after discharge for compliance with the hospitals bylaws, federal and state requirements. Assures that patient’s records are uploaded in a timely and accurate manner to the Electronic Health Record (EHR). Communicates with physicians about delinquent reports. Assures the completion of incomplete documentation within the period as specified by the medical staff rules and regulations. Is familiar with requirements and state guidelines for documentation, such as the history and physical, operative report, progress notes and discharge summary. Responsible for the medical record content, maintenance, and retention of records.

DUTIES AND RESPONSIBILITIES:

1.    Demonstrates the ability to handle the key responsibilities below:

•    Record Management: Organize, index, and file patient medical records in both electronic and paper formats.

•    Data Entry: Input patient information into electronic health record (EHR) systems.

•    Confidentiality: Ensure patient confidentiality by following HIPAA and other relevant regulations regarding the storage and sharing of medical records.

•    Records Retrieval: Retrieve patient records as requested by healthcare providers, departments, or authorized personnel.

•    Document Review: Verify the completeness and accuracy of medical records and notify healthcare providers if discrepancies are found.

•    Compliance: Ensure that medical records comply with legal, ethical, and regulatory standards, including any hospital or departmental policies.

•    Patient Interaction: Assist patients or their representatives with record requests, ensuring proper authorization and safeguarding privacy.

•    Coordination: Collaborate with other hospital departments such as billing, insurance, and healthcare providers to ensure proper documentation is available for treatment, billing, and claims purposes.

•    Maintaining Systems: Update and maintain both manual and electronic filing systems.


2.    Collect all discharged patient charts from the patient care units on a daily basis. Analyzes the medical records of discharged patients for accuracy according to department and hospital policies and procedures. Assured all required medical forms including H&P and surgical notes, are properly completed and filed. 

3.    Handles the release of medical records in accordance with hospital policies, legal requirements, and confidentiality standards. Assures timely and accurate handling of medical record requests from patients, healthcare providers, and authorized third parties while safeguarding patient privacy and compliance with HIPAA and other regulatory guidelines.

4.    Regularly review and interpret hospital and departmental policies related to health information management. Ensure health information practices and procedures align with institutional standards, including those related to confidentiality, data security, and record maintenance. Provide feedback to management on the effectiveness of existing policies and recommend improvements to streamline workflows, improve data integrity, and maintain compliance.

5.    Effectively manages time and workflow by prioritizing tasks, staying organized, and meeting deadlines. Demonstrates the ability to handle multiple responsibilities simultaneously while maintaining attention to detail and quality of work. Adapts to changing demands and efficiently allocates time to ensure all tasks are completed in a timely manner.

6.    Reports to work on time and as scheduled. Participates in educational programs, in-service meetings and completes annual educational requirements.

7.    Treats patients and their families with respect and dignity; ensures confidentiality of patients’ records. Maintains patient confidentiality at all times.

8.    Maintains good working relationships both within the department and with other departments. Communicates appropriately and clearly with co-workers, supervisors, managers, nurses and physicians.

9.     Participates in educational programs, in-service meetings and completes annual educational requirements.

10.    Accepts and completes any other duties as assigned or requested.

Qualifications

Education/Certification: High school diploma or general education degree (GED). Certification as a Registered Health Information Technician (RHIT) from the American Health Information Management Association (AHIMA).

Experience:   Previous experience in a healthcare setting, particularly in medical records management, is a plus.

Equipment/Technology:  Ability to use hospital Electronic Health Record (EHR), and operate basic computer functions and software.
