Mountain View Hospital is looking for a Social Worker/Discharge Planner to join our team!
Job Summary: Supervises the performance of discharge planning in accordance with all state and federal mandated regulations in order to evaluate the necessity, appropriateness and efficiency of medical services provided. Maintains compliancy with regulation changes affecting utilization review and case management. Reviews patient’s records evaluates patient’s progress and performs continuing review for identification and need of on-going hospitalization. Reviews and validates physician orders are carried out, reports progress and unusual occurrences on patients to appropriate departments. Ensures cost effective and appropriate health care services are provided to all patients regardless of patients funding. Maintains review and appeal logs, participates in interdisciplinary collaboration with professional staff. Facilitates educational programs and advises physicians and other departments of regulations affecting utilization review. Supervises staff and consults with social services collaborating with other departments in discharge planning. Supports clinical improvement activities of Mountain View Hospital by providing quality review. Performs administrative duties; receives and generates a variety of reports as needed. Assesses the physical, functional, social, psychological, environmental and financial needs of patients. Supervises referrals to appropriate community resources and assesses patient’s support systems. Acts as a patient advocate by assisting in acquiring appropriate medical, nursing or other health care in the home or clinic setting as needed upon discharge. Maintains accurate patient records, ensuring patient confidentiality. Supervises and Performs case management activities of assessing, planning, implementing, coordinating, monitoring and evaluating the need of patients for services to meet health needs both during and after hospitalization. Supervises the discharge planning staff under the direction of Social Services manager.

DUTIES AND RESPONSIBILITIES:

1. Works closely with various departments and physician’s office staff, regarding Utilization Review and Discharge Planning principles.
2. Interface with physicians and hospital staff on Discharge Planning issues.
3. Works directly with administration in establishing policies regarding discharge planning
4. Develop and present educational material for staff and administration on policies and regulations under the terms of participation for Medicare and Medicaid.
5. Collect information on the patient’s level of functioning and personal living situation in order to develop an action oriented case management plan that is time specific to best assist the patient during their hospital stay and upon discharge from the facility. This function will be shared by the supervisor of the department and the staff.
6. Supervise the execution of the case management plan by incorporating a multi-disciplinary team to achieve the desired outcomes for the patient.
7. Monitors the case management plan, and makes changes as needed, to achieve the desired outcomes for the patient, with the assistance of the utilization review / case management staff.
8. Supervises the arrangement of appropriate services for the client, both during the patients stay and after discharge, utilizing all resources.
9. Monitors the patient during their hospital stay, and identifies specific needs and intervenes on the patient’s behalf when needed.
10. Supervises the Discharge planning staff under the direction of the Social Services Manager.
11. Brings to the attention of the staff issues and concerns with Discharge Planning Team.
