Mountain View Hospital is looking for a Practice Transformation Specialist to join our team!

 

JOB SUMMARY:

The population health practice transformation specialist is involved in supporting MVHs initiatives toward clinical integration and accountable care strategies to achieve the triple aim in healthcare: 1) improving quality and patient satisfaction, 2) improving outcomes and the overall health of the population, and 3) reducing the total cost of care. The practice transformation specialist will monitor trends in overall health and wellbeing of the populations we serve. You will be involved in planning for the implementation of best practices in population health with the end-goal of improving patient health outcomes while decreasing healthcare spending within our organization and affiliated clinics. Our objective for the practice transformation specialist is to build strong relationships with providers in the community and our affiliates and have an extensive knowledge of the contracts held by our organization in an effort to ensure we exceed quality and financial goals.

DUTIES AND RESPONSIBILITIES:

  1. Understand all contract requirements and regulations in order to be a resource to the clinic staff for related questions.
  2. Collaborates with other members of the Population Health Team and Clinic Manager to provide the clinic with resources, ideas, and suggestions on implementation.
  3. Understand outpatient clinic operation and build relationships with staff and providers in the clinics.
  4. Assist with data entry and chart audits to close care gaps.
  5. Review, monitor, and engage clinics in care gap closure for all payers.
  6. Identify and address opportunities for optimized clinic growth and utilization impact.

Qualifications

Education/Certifications:  Bachelor’s degree in related field preferred

 

Experience:  1 year required healthcare experience, 2-5 years healthcare experience preferred

 

Equipment/Technology:  Experience with Word, PowerPoint, and various EMR systems. Proficient in Excel and Microsoft Office.

 

Language/Communication: Ability to read and interpret documents such as quality measure implementation guidelines and requirements as well as utilization reports.  Ability to speak effectively before groups or employees of the organization. Must be patient, courteous, compassionate, and respectful with co workers, physicians, and administrative staff. Have the ability to work in a dynamic environment where numerous projects occur at the same time, organization and attention to detail is a must. Ability to communicate medical terminology and relate well with physicians, staff and management.

 

Interpersonal:  Ability to work well with others as a team player. Organized, detail oriented, and the ability to multi-task while maintaining a heavy load of various tasks. Maintain a neat and professional appearance and workspace.

 

Performance:  Must be able to handle conflict issues with staff. Must follow established recommendations and guidelines. Must meet deadlines as needed.
