Mountain View Hospital is looking for a Social Worker/Discharge Planner to join our team!

 

Job Summary:

Assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties. Prepare patient and family for discharge from acute care hospital setting.

DUTIES AND RESPONSIBILITIES:

  1. Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem.
  2. Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations.
  3. Provides service planned to restore patient to optimum social and health adjustment within patient's capacity.
  4. Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability.
  5. Prepares patient histories, service plans, and reports.
  6. Participates in planning for improving health services by interpreting social factors pertinent to development of program.
  7. Assist with patient emotional support of patient and family members as needed.

Qualifications

Education/Certification:  Must be a Licensed Social Worker, Bachelor's degree (B.A.) in social service from four-year college or university; Preferred experience in a healthcare facility.

 

Experience: 3+ years of clinical social work experience.

 

Equipment/Technology:  Basic computer knowledge.

 

Language/Communication: Effective written and oral skills. Ability to comprehend and follow detailed instructions. Ability to effectively present information and respond to questions from groups of managers, patients, customers, and the pressure during difficult circumstances. Speaks clearly and persuasively in positive and negative situations; listens and gets clarification. Ability to be cheerful, trustworthy, loyal, organized and demonstrate professional behavior at all times.

 

Mathematical Skills:  Business related math.

 

Mental Capabilities:  Ability to organize, recognize and correct inefficiencies.  Handle and prioritize multiple requests. 

 

Interpersonal:  Can remain pleasant under extreme pressure and during difficult circumstances.  Ability to be cheerful, trustworthy, loyal and organized.

 

Reasoning Ability:  Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.  Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

 

Performance:  Maintains strict confidentiality, exactness with great attention to detail, and ability to work with others.  Ability to remain politically neutral.  Customer Service oriented.
