Mountain View Hospital is looking for a Referral Management Supervisor to join our team!

 

JOB SUMMARY:

Oversees daily operations of the population health referral management team to ensure smooth operations and to provide patients with the best possible care experience from Mountain View Organization. Coordinates internal and external communications among staff, patients and providers while providing effective and efficient services to patients.

DUTIES AND RESPONSIBILITIES:

  1. Supervises, trains, orients and evaluates performance over assigned personnel. Recommends merit increases, promotions, disciplinary actions.
  2. Responsible for day-to-day operations of department including scheduling issues and makes necessary adjustments in work assignments. Any other duties or task for smooth department operation. Will help to resolve employee issues and will ensure appropriate communication with other managerial staff and departments within the hospital and within the network. 
  3. Helps maintain records of vacation, sick leave, and overtime.
  4. Excellent communication and customer services skills.
  5. Maintains positive relationships with providers and staff within the family practice clinics as well as specialty care providers and all those within network.
  6. Self-motivated individual with strong organizational and leadership skills. Must set priorities and use time effectively.
  7. Flexibility to handle fluctuating workload.
  8. Strong background with population health, health insurance guidelines, and quality measures for all payors.
  9. Ability to work well with others; be a positive example and motivating personality within       the department.
  10. Work with director and manager for updating, creating, & reviewing Policies & Procedures for Population Health initiatives. Collaborate on training/education for department team members. Conduct/participate in regular meeting with staff
  11. Participates in educational programs, in-service meetings and all other meetings as required.
  12. Answers inquiries regarding patient referrals and maintain patient confidentiality at all times while following HIPAA guidelines
  13. Responsible for knowing hospital policies and being familiar with hospital services. 
  14. Provide support to office personnel in the performance of their duties; perform similar and incidental duties as assigned by manager. Any other Duties as deemed necessary.

Qualifications

Education/Certification:  High school diploma required. Degree in healthcare administration preferred.

 

Experience: Minimum of two years related experience necessary in order to gain knowledge of scheduling processes and procedures.  Minimum two years customer service experience preferred. Minimum one year experience in a medical clinic preferred or sufficient additional education.

 

Equipment/Technology:  Must be able to use Microsoft Office, Word, Powerpoint, Excel, Microsoft Office, PowerBI and various EMR systems.

 

Language/Communication:  Must have the sensitivity, maturity, and ability to communicate clearly and concisely, both verbally and in writing. Ability to speak effectively before groups of customers or employees of the organization.  Fully shares information and expresses ideas, opinions and reaction constructively.  Seeks feedback and gives others constructive feedback

 

Mathematical Skills:  General mathematics.

 

Mental Capabilities:  Can concentrate for long periods of time in order to complete assigned tasks with a better than average attention span in order to listen, read and remember verbal and written instructions. Can understand and act upon input from the manager concerning errors and instructions.

 

Interpersonal:  Ability to instruct and work well with others as part of a team.

 

Performance:   Full time position. Ability to assist in conflict resolution with both patients and staff members. Must be able to pay attention to detail while having a focus on deadlines.
