JOB SUMMARY:  Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Teton Cancer Institute and in the community. Facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians.

 

DUTIES AND RESPONSIBILITIES:

Oncology Clinical Care Navigator

 

  1. Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
  2. Serve as an essential link between patients and all other care providers.
  3. Facilitate appointments for consults and support services within established service standards, including social workers, dietitians, physician, etc.
  4. Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
  5. Develop patient education programs and tools.
  6. Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources.
  7. Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff, etc.).
  8. Follow patients throughout the care continuum, including inpatient admissions and collaborate with inpatient management resources.  

Qualifications

QUALIFICATIONS:   

Education/Certification:  Active, unencumbered Idaho license RN, BSN, and OCN preferred, Biotherapy/Chemotherapy Certification through ONS is necessary, current BLS, ACLS and PALS certificates.

Experience:  Experience in oncology or patient care navigation a plus.

Equipment/Technology:  Ability to successfully and competently utilize computer software; Microsoft Office competency required

Language/Communication:  Requires excellent leadership, organizational, written, and verbal communication and excellent interpersonal skills.  Must be able to work in a self-directed environment, with an ability to work with and lead teams

Mathematical Skills:  Moderate math skills required.

Mental Capabilities:  Ability to multi-task with competing priorities, exercise independent judgment with attention to detail.  Must have organizational skills to prepare, assemble and review various documents. Requires problem solving, decision making and critical thinking.

Interpersonal:  Ability to encourage group process and foster growth in others.   Able to accept, clarify and utilize constructive criticism. Excellent presentation skills; ability to implement professional and community based education programs.

Performance:  Can maintain confidentiality.  The ability to work efficiently and effectively without close supervision.    Able to deal with difficult client situations and emergencies in a calm and professional manner.
