Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. Program Description: Our vibrant Older Adult Center helps older members of the community live life to the fullest by providing case assistance, support services and activities to more than 3,000 center members. Birthday, holiday parties and other social events strengthen the sense of community. Our hot lunches provide, vital nutrition support at an affordable price. Our seniors are actively involved in the center, so services and activities reflect their needs and interests.

 

Position Summary: The Aging Service Case Manger provides case assistance to seniors 60+ living throughout New York City. The Case Manager is responsible for assisting and advocating for seniors and continually assesses their individual needs. The Case Manager is responsible for properly documenting services provided in case files ensuring compliance of program policies and funding guidelines.

 

Schedule: Monday 8:00 am to 4:00 pm

 

Salary Range: $51,446.40-$   54,691.00

 

Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

Roles, Responsibilities, and Essential Duties

  • Provides case assistance services to seniors 60+
  • Assist and advocate for seniors on a wide range of issues, including benefits, and entitlements, health, mental health care, housing
  •  Perform occasional assessments and make home visits to homebound seniors to identify their needs, link them to services, and provide ongoing social contact
  •  Prepares documentation (assessments, case logs, case notes, etc.) as stipulated by contractual and agency standards and procedures.
  • Prepares reports, as required
  •  Plan and implement groups/ support groups in the older adult center to meet the evolving needs of the aging population
  • Attends all required trainings
  • Performs other duties as required.

 

 

Qualifications/Educational Requirements

  • BSW degree, or BA/BS degree plus one year supervised casework experience.
  • Experience working with older adults

 

 

 

Skills, Knowledge and Abilities

  • Bilingual English/Spanish preferred

 

Physical Requirements

  • Filing notes and charts
  • Carrying food trays for members

 

 

Work Environment

  • Senior center, and participation in offsite activities
