PurposeThe Direct Support Professional will align his/her scope of work with Jawonio’s Mission, while incorporating Jawonio’s Core Values as the guiding principle for all work activities.

Summary:  Implements program schedule and activities. Performs direct care tasks helping consumers take care of personal needs. Transports consumers and helps them to develop good daily living habits and skills for independence.

Essential Duties and Responsibilities include the following. Other duties may be assigned. 

 

  • Teaches, trains, guides and directs consumers individually and in groups to reinforce skills, learn new skills, and increase independence for attainment of life goals.

 

  • Drives consumers to and from Day Program. Assists consumers moving between home, Day Program and community sites in a manner determined by their personal requirements. Assistance may include physical lifting and carrying, use of wheelchairs, etc., following prescribed safety procedures. Completes vehicle safety checks as required.

 

  • Regularly observes consumers’ behavior and performance. Provides reports, updates, and general observations to the Program Supervisor. May provide direct support to consumers when need is indicated or upon instructions from the Supervisor.

 

  • Assists consumers who are unable to care for themselves. Performs a variety of personal care functions including toileting, dressing, feeding, lifting and moving consumers, as may be required or requested, based on consumer’s individual needs.

 

  • Maintains appropriate documentation. Keeps logs, goal books, and monthly data sheets up to date and accurate. Generates reports or data as needed for program.

 

  • Models appropriate work behavior and performance to consumers on an ongoing basis.

 

  • Keeps program area clean, to ensure a healthy, clean environment for consumers.  Ensures various supplies, equipment and other material used in training are in adequate supply, are stored correctly and in good order.

 

  • Maintains orderly, safe working area and work practices.

Qualifications

Qualifications:

Education/Experience:

High School plus one year experience with similar population, or equivalent combination of education and experience.

 

Language Ability:

Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to consumers and employees of the organization.

 

Math Ability:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

 

Reasoning Ability:

Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.

Supervisory Responsibilities:

This job has no supervisory responsibilities.

Additional Requirements:

  • A valid NY State Driver’s License and ability to drive and driving record acceptable for agency insurance coverage are required.
  • Remain current in all NYS OPWDD and or OMH and agency required trainings,
  • Candidates must be cleared and maintain acceptable record under the NYS OPWDD and NYS OMH mandated criminal background check process.
  • Must be clear and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process.
  • Must obtain Medication Certification within a time period defined by Program Director.
