As a Case Manager, you will support clients in the Residential System and perform duties in collaboration with a clinician. You will report to the Case Management Supervisor, Residential and you will work onsite with community-based travel..
Responsibilities:
- Arrange and attend home visits
- Prepare transition plans
- Coordinate Independent Living Services
- Be the primary contact between Schools and Residential
- Coach family visits
- Complete various assessments, including FASP and SPR
- Complete accurate case notes and input the notes into Connections database
- Provide transportation for clients
- Maintain contact with DSS and family
- Function as a contributing member of the interdisciplinary treatment team
- Attend dorm meetings
- Attend treatment review meetings
Requirements and Education:
- Requires a Bachelor’s degree or commensurate level of education and related experience.
- Experience with individuals in care systems preferred.
- Must pass physical.
