POSITION OVERVIEW

POSITION SUMMARY

Provide targeted skill building activities with youth in community residence program. Skill training to include areas of independent living and social skill activities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • At minimum, 50% of work week is spent face to face with clients to provide services designated in the Treatment Plan.
  • Provide services based on the youths needs and preferences as identified in the Treatment Plan.
  • Work with treatment team members to identify goals and objectives of treatment, including youth, families, school, and counselors.
  • Complete all required documentation and submit to the Supervisor within required timeframes.
  • Treatment plan goals may include Independent Living or social skills training examples Career Planning, Self-Care, Daily Living, Housing, Money Management, including establishing and maintaining interpersonal relationships, how to accept no as an answer, handling conflict with peers and authority figures and using their coping skills when feeling dysregulated.
  • Staff need to maintain consistency, provide regular routines and be aware of their own emotions when dealing with the youth in care.
  • Engage and form positive relationships with all youth served.
  • Maintain respectful and appropriate communication with internal and external customers, including county workers, medical staff, parents etc.
  • Develop and maintain relationships with community organizations and consumers to expand opportunities for community connections.
  • Facilitate service provision and attend relevant meetings as required.
  • Provide information to collateral contacts for the purpose of Treatment planning and assist in tracking progress as required.
  • Attend all required trainings and meet regularly for supervision.
  • Ability to work with diverse consumers who have mental health challenges and may be developmentally delayed. Understand their independent living needs to work with case manager to find resources in the community for educational and vocational skills and programming for discharge.

EDUCATION and/or EXPERIENCE

  • High School Diploma/GED required
  • At least 2 years of experience

CERTIFICATION / LICENSURE

  • Valid drivers license required
