Licensed Practical Nurse - Full Time LPN - Albany, NY
As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.
Responsibilities:
- Complete the nursing observations report findings to the registered nurse
- Review lab and test results, reporting critical values to RN and providers
- Medication management including ordering medications and obtaining prior authorizations
- Dispense medications
- Communicate with medical director/providers regarding observation in client’s health/behavior
- Provide client and family education regarding medications and treatments
- Collaborate with our staff regarding concerns and behaviors
- Participate in Interdisciplinary Medication Review meetings
- Coordinate, monitor, and document the health care of youth with internal and external providers
- Schedule and assist with psychiatric med reviews for children in the program
- Coordinate referrals made to specialty clinics and follow up on recommended tests, treatments, or therapies
- Complete nursing documentation using EHR including logs, patient contacts, uploading and storing medical documentation, and tracking the attendance and follow ups of medical appointments
- Update discharge planning needs
- Other tasks as assigned within scope of practice and based on agency needsÂ
Requirements and Education:
- LPN Licensed in New York State
- CPR Certification required.
- 1 to 2 years of experience required
- Experience working with youth and adolescent population desiredÂ
- Must pass physical.
- Will complete TCI training.Â
